■■Therapeutisches Management<br />
Die Auswahl der antimykotischen Therapie richtet sich<br />
nach<br />
• der lokalen Epidemiologie,<br />
• den Risikofaktoren,<br />
• der Grundkrankheit und<br />
• den Begleitmedikationen von Patienten<br />
Es gibt folgende grundlegende Behandlungsstrategien:<br />
• Die Prophylaxe zur Reduktion der Infektionsrate von<br />
Risikopatienten ohne Infektionssymptome.<br />
• Die empirische Therapie symptomatischer Patienten,<br />
deren Risiko für die Entwicklung einer invasiven Pilzinfektion<br />
hoch ist.<br />
• Die präemptive Therapie von Risikopatienten, die bereits<br />
klinische, radiologische oder laborchemische<br />
Hinweise auf invasives Pilzwachstum zeigen, jedoch<br />
ohne verlässlichen Beweis<br />
• Die gezielte Therapie beim Nachweis einer invasiven<br />
Mykose.<br />
• Die Salvage (Zweitlinien-)Therapie bei einer nachgewiesenen,<br />
aber therapierefraktären invasiven Mykose.<br />
Da eine verspätete Diagnose mit einer hohen Sterblichkeit<br />
einhergeht, sollte die Therapie einer invasiven Aspergillose/Mykose<br />
bereits vor dem mikroskopischen/histologischen<br />
Nachweis empirisch (präemptiv) begonnen werden.<br />
Bei ausbleibender Entfieberung unter adäquater<br />
Antibiotikatherapie sollte bei Risikopatienten eine entsprechende<br />
Behandlung innerhalb von drei Tagen beginnen.<br />
Die Differenzialtherapie orientiert sich an den nachgewiesenen<br />
oder vermuteten Pilzerregern, den individuellen<br />
Risikofaktoren und der Abwägung möglicher Nebenwirkungen.<br />

bei Schimmelpilzinfektionen gibt Tab. 2,<br />
Seite 11. Die aktuell in Österreich zur Verfügung stehenden<br />
Antimykotika zur Behandlung von Pilzinfektionen<br />
sind in Tab. 3 zusammengefasst.<br />
