plaintiffamended the llling document / form by oëcial notarized notice as exhibit A 1 to<br />

correct wrong information including correction to EEOC form 5(1 1/09) dated Feb. 02,201 1 l<br />

eoibit M . The discriminatory acts reported by plaintil-oflcially was lbr:- l-age z-national<br />

origin 3-religion 4-sex.<br />

2- The director of EEOC advised Plaintiff that plaintiff must wait for six months to get any<br />

information's including a copy from my employer's response when plaintiffrequested b%ed on<br />

freedom of information act FOIA regulations blzr ljust got only my employer's response few<br />

days ago. Plaintiffreceived nothing till now except the EEOC decision titled trismissal And<br />

Notice Of Rights' which I received it on Sep. 07,201 1 by certified mail for the decision dated on<br />

Aug. 26,201 1 exàibit M . W lile the certified mail receipt has two sides one for plaintifrs<br />

signature only the other side wms indicated different name from the address on the envelope<br />

wllich should be match EEOC address. Therefore plaintiff has to reported it offkialy because the<br />

sender name on the receipt which I have to sign is for EEO witich Eqlnl Education Opportunity<br />

in mv emolover not EEO for Enual Emolovment Onoortuniw which plaintiFhas no business<br />

w1t1 both W lile 1St one couldn't provide any assistnnt the 2nd one only to mssist teachers who has<br />

the poskion title. Why this certified mail receipt for signature has diferent address from the<br />

address for EEOC Minmi oxce who did send the mail'?. n erefore I documentcd to my post<br />

oflice and to EEOC oflke which take time to conirmed the problem and to receive the right<br />

mail send by EEOC which has the decision on the charge against respondent.<br />

3- The decision / order by EEOC Miami office was supported plaintifrs case on one point<br />

of 1aw forjob discrimination but in other point of 1aw the ofice reported incorrect information to<br />

the discriminations acts which never filed by plaintiffthese two points in two separate parts as<br />

folow:-<br />
