Adenoma<br />

Definition<br />

A benign tumour of r<strong>et</strong>e epithelial origin<br />

that occurs within the dilated r<strong>et</strong>e and<br />

typically has a tubular pattern resembling<br />

Sertoli cell tumour.<br />

ICD-O code 8140/0<br />

Clinical features and histopathology<br />

This is a rare tumour that mostly occurs in<br />

adults. It typically forms polypoid nodules<br />

composed of tubules that project<br />

into the dilated lumen of the r<strong>et</strong>e testis.<br />

The tubules resemble those seen in<br />

benign Sertoli cell tumours.<br />

Adenocarcinoma<br />

Definition<br />

Recommended criteria for the diagnosis<br />

of adenocarcinoma of the r<strong>et</strong>e testis are:<br />

no histologically similar extrascrotal primary,<br />

tumour centred on testicular hilum,<br />

absence of conventional germinal or non<br />

germinal testicular cancer, histologic<br />

transition from unaffected r<strong>et</strong>e testis,<br />

solid growth pattern {1908}.<br />

Epidemiology and clinical features<br />

R<strong>et</strong>e testis carcinoma is rare, its <strong>et</strong>iology<br />

unknown. The tumour, predominating in<br />

the fourth through eighth decades, is<br />

usually associated with a scrotal mass,<br />

tenderness, or lumbar pain. It may be<br />

masked by an inguinal hernia, hydrocele,<br />

fistula, sinus or epididymitis. Symptoms<br />

are brief or extend over years. Locally<br />

recurrent tumour nodules and abscesses<br />

may involve the scrotal and perineal skin.<br />

A statistical analysis, based on published<br />

data, was reported {2288}.<br />

Macroscopy<br />

The carcinoma usually forms a non<br />

encapsulated firm, pale rubbery hilar<br />

mass. A cystic component, if any, is usually<br />

minor. Reported lesional size ranges<br />
