Foundation Scholarship for Survivors:<br />

Jim Meade and Luke Barlowe Award<br />

Eligibility: Must be a <strong>Kentucky</strong> resident<br />

under the age of 25 and a survivor or current<br />

patient of a diagnosis of cancer or a critical<br />

or life-threatening disease or illness; submit<br />

a completed application, an essay (no more<br />

than 1,000 words) on how diagnosis has<br />

impacted life and future goals, a copy of an<br />

acceptance letter from the college, university,<br />

or vocational-technical school student plans<br />

to attend, and written documentation from<br />

treating physician stating medical history;<br />
