74 3100B HFOV Unexplained Operation Condition Possible Cause Possible Solution Oscillator shuts down and Dump Valve opens during operation Oscillator will not restart after temporary disconnection (such as for routine suctioning). Pa unstable—jumps by 2–3 cmH2O Humidifier not operating properly Pa jumping by > 5 cmH2O when trying to adjust with Pa Adjust Valve. Oscillator making a squeaking sound 1. Drastic change in Pa due to overaggressive control change using the Pa Adjust. 2. ET Tube has become disconnected. 3. Radio Frequency Interference. 1. To restart oscillator, Pa must first be >5 cmH2O, but in order to achieve Pa >5 cmH2O, oscillator must be on. 1. Water collecting at Pa Control Valve. 2. Patient spontaneously breathing. 3. Worn or defective cap diaphragm. 4. Internal Failure. 1. Excessive heat from driver 2. Room temperature > 84°F 1. Worn or improperly seated cap diaphragm. 2. Internal Failure. 1. Re-establish Pa and make any small adjustments to Pa using Flow-meter Adjust. Note: see Clinical Guidelines chapter for minimum flow requirements. 2. Reconnect ET Tube. 3. Locate and distance offending device. 1. Reduce power and increase Pa to target level using flow meter and Pa Adjust Control—then increase power while keeping Pa on target by adjusting flow meter or Pa control valve down. 1. Adjust circuit height for better draining. 2. Bias Flow rate possibly insufficient; readjust Pa using higher flow. Also, consider clinical status of patient. 3. Replace cap diaphragms. 4. Call CareFusion. 1. Ensure cooling gas is connected. Try different source connection for cooling gas. 2. Decrease room temperature. 1. Replace cap diaphragms. 2. Call CareFusion Service. 1. Cap diaphragm defective. 1. Replace cap diaphragm. 767164–101 Rev. R
