It is now seven years since the originalbook appeared, and there have been many moreadvances in the subject. In spite of new sectionsand some wholesale rewriting, it has beenpossible once again, to contain the informationin one volume. Very sadly some of our originalcontributors have died. New authors havereplaced them while others have been added inan endeavour to keep the text authoritative andup-to-date. The helpful criticisms of the firstedition have been carefully considered in thepreparation of this one. Having so manydistinguished authors with such a breadth ofinterest, while greatly enhancing the book, hasled to a long gestation period, but we believe thatit has been worthwhile. Much of the originalformat has been retained in order to continue tostimulate lively debate and exchange of views.
