She was in a group of three divers whowere dropped off by a boat. Simultaneously,the dive boat was tending to several groups ofdivers in different areas. The current was verystrong, and the divers felt conditions were notsafe for making the dive. The dive boat pickedup the other two divers in the group, but thedecedent had disappeared below the surface,and her body was never recovered.03-16 Experienced divemaster with chroniccardiac problems, signaled problems on thebottom and disappearedCause of Death: Unknown Cause of Death,Body Not RecoveredThis 58-year-old male was a very experienceddivemaster and a dive shop owner. He had amedical history significant for severe coronarya t h e ro s c l e rosis, including a previous stentplacement. The decedent had been taking severalcardiac medications, including nitroglycerin.After diving the previous day, he hadcomplained of shortness of breath. The diverand his buddy descended to 70 fsw (21 msw);after approximately 10 minutes he signaled tohis buddy that he did not feel well, and theybecame separated. The diver’s buddy ascendedalone, but he did not see the missing diverat the surface. A body was never recovered.03-23 Inexperienced diver, surfaced far fromboat, separated from buddy on their underwaterswim back and lostCause of Death: Unknown Cause of Death,Body Not RecoveredWhile this 29-year-old female had advancedopen-water certification, she had made fewerthan 20 dives since initial certification threeyears before. The dive profile was not available,but she and her buddy surfaced far fromthe boat and descended again to swim back.The buddy became low on air, and both diverswere fatigued. They became separated, andthe buddy was rescued on the surface a fewhours later. The decedent’s body was neverrecovered, though some of her equipment wasfound 13 days later.03-30 Experienced diver lost in open-waterdiveCause of Death: Unknown Cause of Death,Body Not RecoveredThere are few details available regarding thedeath of this 75-year-old male who was anexperienced, certified diver. He never surfacedfrom an open-water dive. His body wasnever recovered.03-41 Experienced technical diver, problemwith buoy during ascent, separated frombuddy, body never recoveredCause of Death: Unknown Cause of Death,Body Not RecoveredThis 60-year-old male, who was a very experiencedtechnical diver, had made a wreck penetrationdive using nitrox and a drysuit.During the ascent phase of the dive, there wassome problem putting up a dive buoy. Thediver’s buddy motioned for the decedent tojoin him at the 40 fsw (12 msw) decompressionstop, but then they became separated. Thedive buddy returned to the boat, and the bodyof the missing diver was never recovered.03-57 Divemaster and buddy lost in underwatercave systemCause of Death: Unknown Cause of Death,Body Not RecoveredThis 22-year-old female was involved in adouble diving fatality (there were an unusuallylarge number of double fatality cases in2003). She was also on antidepressantmedications. Her body was never recovered.03-58 Experienced dive instructor withunknown cave diving certification, planneddive to underwater cave system with buddy,both bodies were lost, never recoveredCause of Death: Unknown Cause of Death,Body Not RecoveredThis 24-year-old male was involved in a doublediving fatality. He and his dive buddyplanned a dive into an underwater cave system.This diver was an experienced diveinstructor. It is not clear if he had formal documentationof cave diving training. His bodywas never recovered.03-85 Experienced diver attempted reach400’ on air, body never foundCause of Death: Unknown Cause of Death,Body Not RecoveredThis 21-year-old male was a very experienceddiver with instructor and technical diving certifications.The night before this dive, the decedentwas bragging that he would attempt arecord dive to in excess of 400 fsw (123 msw)on air. These statements were made in associationwith the consumption of alcoholic beverages.The next morning , the diver and twodive buddies made a deep dive on a wall. Thebuddies turned back at the 300 fsw (92 msw)mark, but the decedent continued to descend.His body was never recovered.03-05 Open-water dive, limited informationCause of Death: Unspecified Cause ofDeathThere is virtually no information available onthis diving death that involved a 65-year-oldfemale. She was apparently making an openwaterdive in the ocean. Her certification status,level of experience and the circumstancessurrounding the event are unknown.03-80 Diver with history of breast cancer,made ocean dive, limited informationCause of Death: Unspecified Cause ofDeathVirtually nothing is known about the circumstancessurrounding the death of this 44-yearoldfemale. She had a history of treated breastc a n c e r. Her certification status and divingexperience are unknown. The investigativereport states that the death involved an oceandive, but over a year later, that the death wasstill under investigation. No additional informationis available at this time.03-84 Inexperienced diver, low on air, lostconsciousness at surfaceCause of Death: Unspecified Cause ofDeathInformation about the death of this 44-yearoldmale is incomplete. He possessedadvanced open-water certification but hadonly made 20 lifetime dives over nine years.The diver had a history of back pain for whichhe took a muscle relaxant , and he may havebeen drinking alcoholic beverages the nightbefore this dive trip. During the second dive ofthe day, the decedent ran low on air and surfacedwith his buddy. He then lost consciousness.A cardiac event was suspected, but an airembolism is every bit as likely. If an autopsywas performed, the report was not madeavailable for review.03-89 Diver collapses after shore dive, limitedinformationCause of Death: Unspecified Cause ofDeathThere was virtually no information availableon circumstances surrounding the death ofthis male whose age is unknown. It cannoteven be said with certainty that the death wasdive-related.
