General Information 15· Prior Breast Surgery4. Select one of the following options:· Copy to copy previous profile data.· Exam Data to access the next record category.5. Complete the Patient Profile Record with the appropriate information:· Menarche Age: Enter the age the patient began menstruation. (No entry is needed for malepatients.)· Age at First Birth: Enter the age of the patient when her first child was born or “N” fornulliparous. (No entry is needed for male patients.)NOTE: Enter “U” for any of the following fields that do not apply.· Still Menstruating: Enter Y (yes), N (no), or U (unknown).· Hysterectomy: Enter Y (yes), N (no), or U (unknown).· Patient on Hormones: Enter Y (yes), N (no), or U (unknown).· Past Menopause: Enter Y (yes), N (no), or U (unknown).· Family History: Enter Y (yes), N (no), or U (unknown).· Clinical Symptoms: Enter Y (yes), N (no), or U (unknown).Clinical Symptoms is a required question. The Y (yes) or N (no) response entered in thisfield (N = screening, Y = non-screening) determines whether the mammogram isSCREENING or NON-SCREENING.CAUTION: It is recommended that U (unknown) is used with caution. Records that have a “U” inthis field will not be included in the QA Summary Report as screening or non-screening statistics.· An entry of Y (yes) or N (no) in this field is required for the system to calculateTrue/False/Pos/Neg.· The calculation of True/False/Pos/Neg is required for the mammogram record to beconsidered resolved.· The calculation of True/False/Pos/Neg is what determines the mammogram record to beresolved.· The QA Summary Report provides statistics on resolved records only.• Palpable Finding - Enter Y (yes), N (no), or U (unknown).· An entry of Y (yes) or N (no) in this field is required for the system to calculateTrue/False/Pos/Neg.© 2009 Computer Programs and Systems, Inc.
