22<strong>Mammography</strong> <strong>Workbook</strong>Additional Evaluation RecommendationsNOTE: It is not necessary to enter information in this record category if the assessment code isB-Benign or N-Negative. This record category will be completed automatically when the patient’s assessment code is B-Benign or N-Negative and the Patient Profile Record and Exam Datacategories are complete.The <strong>Mammography</strong> System Additional Evaluation Recommendations category consists of 10fields including a comment field to document additional information. Each facility has the option todesignate any or all of these fields as required patient information for the mammogram record.Therequired fields are defined in the <strong>Mammography</strong> Data Fields Control table.The Additional Evaluation Recommendations will remain incomplete until:· All fields have been addressed with a Y (yes) or N (no) response, (N must be entered foroptions that are not recommended) and/or· Both of the previous record categories have also been completed.· If Y (yes) has been entered in any of the additional evaluation recommendation fields, the entiremammogram record will remain incomplete until the corresponding Additional Evaluation Datafield has been completed with one of the following options:• Y (yes) and the date of the procedure.• N (no) if the patient did not return for the additional procedures.1.
