I understand the importance of my participation in the proper use of this medication.Taking prescribed dose as directedHaving blood work (PT/INR) done on time as ordered by physicianNotifying my MFHC Provider if I start any new medications or supplements as these maychange my warfarin level, requiring additional testing of my PT/INRKeeping in close contact with physician concerning blood test results (PT/INR) and possibleadjustment of warfarinContacting the clinic for results and further directions if I have not heard from the clinic within aday of having blood work (PT/INR) (All blood draws should be done Monday throughThursday, avoiding the day before a holiday or weekend, unless special arrangements have beenmade)Seeking medical attention for any signs of bleeding, trauma or significant changesKeeping my physician informed of my current phone numberGiving permission to leave messages concerning my PT/INR and warfarin dosage on myanswering machine (or my contact person’s answering machine) if necessarySeeing my primary care doctor a minimum of every six (6) monthsI have read, understand and agree to the above.Patient Signature ____________________________________Guardian Signature __________________________________Date ______________________________________________Witness ________________________________________DUTIES OF THE CLINIC I DOCTORDuring this rotation the resident will be in clinic from 8:00 a.m. to 5:00 p.m. daily,Monday - Friday. The resident will be responsible for all specialty clinics as scheduled.They are also responsible for follow-up on all patients seen in specialty clinics duringtheir month of clinic rotation.When not scheduled for a specialty clinic, the resident will see his/her continuity patientsand/or work-ins. When post-call, the resident will have the afternoon off (but call must bescheduled so that required specialty clinics are covered). Other health centerresponsibilities vary according to the Track and may include checking charts, daily lab,and prescription refills of those residents on away electives and vacation, as outlinedbelow.1. Review MFHC lab reports.a. C1 will review MFHC lab buggy daily from 8-9am (time is allotted for this).b.
