Objective
to determine the prevalence, adherence to therapy, attainment of blood pressure (bp) targets and
cost of therapy in patients with coexisting hypertension and type 2 Diabetes Mellitus (t2DM)
Setting
out patient Diabetic Clinic in a university affiliated teaching hospital
Methods
using a structured questionnaire details regarding duration and type of Diabetes Mellitus, therapy,
co-morbidities, compliance and cost of therapy was ascertained. blood pressure measurements
on two occasions 15 minutes apart and anthropometry was obtained by the investigator. targets
for systolic and diastolic blood pressure were based on american Diabetes association (aDa)
standards of care.
Results
hundred patients were interviewed. sixty two percent had coexisting hypertension. prevalence of
hypertension was more in females (70% p=0.005). the body Mass index (bMi) among patients with
coexisting hypertension was significantly more than among those without hypertension (28.6±5.2
vs 26.6±3.2 p= 0.03). Co-morbid coronary artery disease (74% vs 26% p=0.001), chronic kidney
disease (64% vs 36% p=0.001) and Dyslipidemia (95% vs 35% p=0.001) was significantly more in
patients with coexisting hypertension and t2DM. only 6.2% of the hypertensive patients had bp
targets meeting the systolic and diastolic goals prescribed by the aDa. Compliance to drug therapy
was good in around 75% in patients with and without hypertension (77.8% vs 78.9% p=ns). Cost
of therapy was 2.2 times higher in patients with coexisting hypertension with t2DM (rs 8191 vs
rs 3698 per year)
Conclusions
two in three patients with t2DM have coexisting hypertension. the prevalence of hypertension
is much higher in women and the obese. Despite significant co-morbidities only 6% of patients
with hypertension achieved bp targets in a university affiliated Diabetic Clinic.
