<strong>Study</strong> of Silent Myocardial Ischemia In Type 2 Diabetic Males<br />

<strong>in</strong>troduction: accelerated coronary and peripheral vascular atherosclerosis is one of the most<br />
common and chronic complications of diabetes mellitus. a recent aspect of coronary artery disease<br />
<strong>in</strong> this condition is its silent nature.<br />
aim of the work: is to detect the prevalence of silent myocardial ischemia (sMi) <strong>in</strong> type 2 diabetic<br />
males <strong>in</strong> our locality and to select male diabetic population who should be screened for sMi.<br />
patients and methods: the study was carried out on 100 type 2 diabetic males with negative<br />
history of ang<strong>in</strong>a and 30 healthy males. Laboratory estimation of f bG level, lipid profile , (hba1c),<br />
microalbum<strong>in</strong>urea, and C-reactive prote<strong>in</strong> were done for all participants. non <strong>in</strong>vasive tests (nits)<br />
<strong>in</strong>clud<strong>in</strong>g 12 leads rest<strong>in</strong>g eCG, trans-thoracic echocardiography, treadmill exercise eCG, myocardial<br />
perfusion imag<strong>in</strong>g were done for all participants and patients positive for one or more nits were<br />
subjected for coronary angiography.<br />
results: (29%) were positive for one or more nits <strong>in</strong> the patients group Vs (3.3%) <strong>in</strong> the control.<br />
after coronary angiography, 20 patients were positive for significant coronary artery stenosis.<br />
there were statistical significant difference between the two groups as regard family history of DM<br />
and ihD, hypertension, and obesity with higher levels of microalbum<strong>in</strong>urea, C-reactive prote<strong>in</strong>,<br />
total cholesterol, and triglycerides <strong>in</strong> the patients group Vs control.<br />
Conclusion: type 2 diabetic male patients should be screened for detection of sMi when age above<br />
50, diabetes duration is more than 5 years , presence of two or more cardiac risk factors and/or<br />
patients suffer<strong>in</strong>g from one or more of the chronic diabetic complications.<br />
