What does the evidence say about the fetal and neonatal safety about the use of oral Ant<br />

Diabetic Drugs (aDD) <strong>in</strong> pregnancy and Lactation<br />

objective : to review the safety of oral anti diabetic agents <strong>in</strong> pregnancy and Lactation<br />

Glyburide and metform<strong>in</strong> are not teratogenic <strong>in</strong> humans when used <strong>in</strong> cl<strong>in</strong>ically<br />

recommended doses.<br />

Glyburide may be used for treatment of gestational diabetes <strong>in</strong> some women.<br />

Metform<strong>in</strong> may be used safely for ovulation <strong>in</strong> women with pCos.<br />

Metform<strong>in</strong> reduces the development of GDM, was not teratogenic and did not affect<br />

birth length, weight, growth motor or social development <strong>in</strong> children <strong>in</strong> first 18 months<br />

of life<br />

tolbutamide, Chloropropamide cross <strong>in</strong>to the breast milk<br />

None of the glyburides pass <strong>in</strong> the breast milk samples.<br />

MetforM<strong>in</strong>: excreted <strong>in</strong> the breast milk at very low levels<br />

no differences <strong>in</strong> weight, height or motor-social development of the newborn or <strong>in</strong> the<br />

<strong>in</strong>fants up to 3-6 months of age are reported.<br />

there is evidence that certa<strong>in</strong> oral hypoglycemic drugs might be safe <strong>in</strong> pregnancy, or<br />

at least, are significantly less dangerous to the develop<strong>in</strong>g fetus than poorly controlled<br />

diabetes.<br />

Metform<strong>in</strong>, Glyburide and Glypizide are compatible with breast feed<strong>in</strong>g<br />

these agents may be used <strong>in</strong> research sett<strong>in</strong>gs<br />

if the patient needs <strong>in</strong>tervention but refuses <strong>in</strong>sul<strong>in</strong> therapy,<br />

patient is aware of potential or probable risks<br />
