PRACTICE SPOTLIGHT Total health care for seniors by Larry Schwingel Glades Medical Group (GMG) is a practice that began in Boca Raton decades ago under the supervision of David B. Hevert, M.D. Providing exceptional care to residents, the group practice is one of the area’s gold standards of service. Dr. Hevert is joined on the team by physicians Randall Taubman, M.D., Jorge I. Montalvan, M.D., and Janice S. Plaxe, D.O. Their combined experience equates to nearly three quarters of a century. An internal medicine and geriatrician specialist, Dr. Taubman said physicians at GMG treat a wide age range of patients ranging from 18 years to those at the end stages of life. “Our focus is on preventive medicine and wellness; I like to call it ‘head-to-toe’ care,” he said. “Each patient is unique and different, so we take great care to chart a treatment plan best suited for their individual needs.” This long-standing practice has a vested interest in each patient, even when they are hospitalized. Many group practices designate hospital care to another service provider, but that is not the policy of GMG. “We want to make sure our patients receive the best continuum of care possible, so we are in the hospital routinely to make sure that this happens,” Dr. Taubman said. His certification as a geriatrician enables him to understand the unique needs of the elderly, as they may have chronic or acute medical conditions such as heart disease, diabetes, lung disease, gastrointestinal issues, or other problems. “Elderly-patient care often involves multiple health concerns at the same time, and they may not respond to treatments the same way as younger patients,” remarked the doctor. “Geriatricians are trained to deal with multiple illness situations and we also can evaluate social aspects such as home support.” According to the doctor, GMG works closely with a variety of service providers including home health, physical therapy, and wound care. The specialist is also trained to detect complications from diseases and more mild problems, such as dehydration. A mild problem for the elderly can compound a situation that will lead to more serious health consequences. For example, a fever may cause confusion that leads to a fall that can cause a possible bone fracture. “It’s what we call a constellation effect,” Dr. Taubman said. The most common grouping, according to the specialist, is the metabolic syndrome combination of hypertension, diabetes, obesity, and hyperlipidemia (a condition where too many lipids or fats are in the blood, which leads to high cholesterol and high triglycerides). Diabetics may have up to two or three manifestations or complications. These include neuropathy, kidney disease, eye disease, and heart disease. “The older one gets, the chances for these conditions increase. It’s important to keep a regular schedule of checkups,” he said. If an elderly person is having a harder time managing a chronic condition, it is time to see a geriatrician. “Heart issues, diabetes, emphysema, asthma, or other conditions require a higher degree of surveillance and we provide that,” Dr. Taubman stated. One of the biggest challenges is helping senior patients understand that annual checkups or periodic care they received when they were younger is no longer the solution. “That approach does not work as we age,” said the geriatrician. “Elderly patients have between four and six chronic conditions, so a more balanced schedule of care is needed; at the very least, a quarterly visit.” “As specialists, we at GMG take the time to care,” Dr. Taubman said. “We take the time to peel back the layers of their medical history and actually listen to understand our patient’s needs.” For more information, visit gladesmedical.com.
