HEALTH Submitted by Broward Health The ABCs of Stopping a Bleed Could Save a Life The unspeakable tragedy at Marjory Stoneman Douglas High School has been another wakeup call that the world has changed and people need to be better prepared to help before first responders arrive. Stop the Bleed is a national campaign launched by the American College of Surgeons to instruct the public how to apply lifesaving tourniquets and wound seals to increase odds of survival after suffering a traumatic injury, whether it be in a mass casualty situation or at home or on the playground. “We have this concept; it’s called the golden hour,” said Igor Nichiporenko, M.D., medical director of trauma services at Broward Health North. “We have 60 minutes to save someone’s life. If we teach a regular, non-medical person how to stop the bleed, we can save a lot of lives.” Nichiporenko, who played a major role in treating many of the Stoneman Douglas shooting victims, said the Stop the Bleed tools and techniques employed by first responders in Parkland helped to save many lives. A recent Stop the Bleed training at Broward Health Medical Center in Fort Lauderdale drew a cross section of about 100 community volunteers and public-spirited citizens, including many who work in security or other frontline public positions. The two-and-a-half hour course focused on bleeding control to prevent death from hemorrhage, including how to identify life-threatening bleeding injuries and how bystanders can help to control bleeding until emergency responders arrive at the scene. It covered the ABCs of bleeding. On its face, the concept seems simple. “A” is to be alert and call 9-1-1. “B” is 102 to find the bleeding injury and “C” is to compress it. But Stop the Bleed training requires careful attention to detail. Arm and leg wounds are very different from “torso junctional wounds” at the neck, shoulder or groin. They are quite distinct from chest and abdominal injuries, with bleeding that cannot be stopped outside the hospital. During the class, hands-on skills training focused on the proper application of tourniquets to stop bleeding of the extremities. In small groups, students were given Combat Application Tourniquets and instructed by Fort Lauderdale Fire Rescue team members how to correctly apply them to their own arms and legs. A second training station taught each person how to reach into a wound to find the bleeding source, then how to stop the bleed with pressure while properly packing the wound. A third station focused on chest and abdominal bleeds, including the proper application of chest seals. In response to numerous mass casualty events, in 2013 the American College of Surgeons first called for training the public in bleeding control techniques. It estimates that as many as 120,000 people have participated in Stop the Bleed training programs throughout the United States. Numerous government agencies and public service organizations are active participants in the national Stop the Bleeding Coalition. According to national statistics published by the Coalition, 35 JUNE <strong>2018</strong> percent of pre-hospital deaths are the result of hemorrhaging and 80 percent of mass casualty victims arrive by nonemergency vehicles. As a result, the Coalition is actively encouraging training and wider use of proven battlefield military medical techniques such as the proper application of tourniquets. Broward Health President and CEO Beverly Capasso said Broward Health is committed to ongoing Stop the Bleed community training programs. “As a former trauma nurse, I can attest that tourniquet training is an invaluable skill for everyone,” she said. “It can be used for a mass casualty incident or singular injury, such as an emergency at home, on a playground or beyond.
