(MAC) has been introduced to streamline medical admissions <strong>and</strong> is additionally<br />

staffed by an on call consultant of the day as well as by permanent admissions<br />

unit staff grades aiding the take. Full radiological, endoscopic, ITU <strong>and</strong> HDU<br />

support is available. Specialist skills provision occurs in the Coronary care <strong>and</strong><br />

stroke units with respiratory expertise for BIPAP. An outreach team from ITU<br />

provides additional expert assistance <strong>and</strong> advice for medically sick patients.<br />

<strong>Diabetes</strong> <strong>and</strong> <strong>Endocrinology</strong><br />

The full range of <strong>Diabetes</strong> <strong>and</strong> Endocrine specialty services are provided. The<br />

main base for the post is at Kingston Hospital, but one diabetes clinic is based at<br />

the newly built Queen Mary’s hospital in Roehampton, a pleasant 10 minutes<br />

away across Richmond Park.<br />

A full range of diabetes clinics run <strong>and</strong> include<br />

specialist adolescent, eye, antenatal <strong>and</strong> renal<br />

clinics as well as multidisciplinary clinics for the<br />

diabetic foot which has established orthotic links<br />

with the Douglas Bader unit. There is senior<br />

experienced diabetes nurse support from a team<br />

that includes one of the first Diabetic nurse<br />

consultants as well as psychology <strong>and</strong> dietetic<br />

expertise. A number of patients in clinic are now<br />

managed with pump therapy <strong>and</strong> a continuous<br />

glucose monitor has been purchased <strong>and</strong> is<br />

available for clinical <strong>and</strong> study use.<br />

The diabetes renal clinic is run jointly with a<br />

consultant nephrologist from St Georges <strong>and</strong><br />

offers opportunities for research with projects<br />

currently running in renal anaemia <strong>and</strong> vascular<br />

stiffness.<br />

The consultants at Kingston have research interests in the pathogenesis of renal<br />

complications, renal anaemia, diabetes <strong>and</strong> pregnancy <strong>and</strong> the investigation of<br />

endocrine disease.<br />

Endocrine clinics are unselected with registrars involved recently in the<br />

investigation <strong>and</strong> diagnosis of acromegaly, Sheehans’ syndrome,<br />

hypoparathyroidism <strong>and</strong> an insulinoma.<br />

Close endocrine surgical links exist with St George’s hospital <strong>and</strong> Queens Square<br />

as well as with the Hammersmith hospital providing endocrine <strong>and</strong> neurosurgical<br />

management for the patients.<br />

Monthly endocrine meetings occur with biochemistry <strong>and</strong> neuroradiology allowing<br />

discussion of ongoing <strong>and</strong> interesting cases.<br />
