<strong>1.</strong> OBIECTIVE GENERALE<br />

Dupa cancerul de plaman, cancerul de prostata (CaP) este cea de-a sasea cauza<br />

de cancer <strong>in</strong> lume (cazuri noi) <strong>si</strong> a treia ca importanta <strong>in</strong> patologia sexului mas<strong>cu</strong>l<strong>in</strong>.<br />

Acest tip de cancer ramane o provocare pentru sanatatea publica <strong>in</strong> ciuda<br />

progresului <strong>in</strong>registrat <strong>in</strong> detectie <strong>si</strong> terapie. Inovatiile <strong>si</strong> aplicarea biotehnologiei au<br />

permis <strong>in</strong>vestigarea proceselor biologice <strong>in</strong> efortul studierii patogenezei la nivel<br />

mole<strong>cu</strong>lar.<br />

Numeroase studii au fost dedicate identificarii markerilor de prognostic care<br />

diferentiaza formele <strong>in</strong>dolente de cele agre<strong>si</strong>ve <strong>in</strong> cancerul de prostata. In contrast,<br />

mult mai put<strong>in</strong>e studii s-au concentrat pe <strong>in</strong>telegerea mecanismelor mole<strong>cu</strong>lare care<br />

stau la baza dezvoltarii prostatei norm<strong>al</strong>e <strong>si</strong> <strong>in</strong>itierii <strong>si</strong> progre<strong>si</strong>ei cancerului la acest<br />

nivel.<br />

Scopul acestui proiect il reprez<strong>in</strong>ta identificarea factorilor de risc genetici <strong>in</strong> cancerul<br />

de prostata, propunand o viziune <strong>in</strong>tegrativa asupra patogenezei <strong>si</strong> evolutiei natur<strong>al</strong>e<br />

a acestei m<strong>al</strong>ignitati, <strong>in</strong> afara oricarui tratament. Se propune stabilirea unor modele<br />

matematice bazate pe rezultatele ev<strong>al</strong>uarii cl<strong>in</strong>ice, anatomopatologice <strong>si</strong><br />

mole<strong>cu</strong>lare, raportate la predictia evolutiei <strong>si</strong> susceptibilitatea famili<strong>al</strong>a <strong>in</strong> cancerul<br />

de prostata pr<strong>in</strong>: an<strong>al</strong>iza IHC a TGF, p53, EGFR, VEGF, AMACR; anom<strong>al</strong>iile<br />

cromozomi<strong>al</strong>e, BRCA1, BRCA2 pr<strong>in</strong> FISH, MLPA, LOH, MSI, <strong>si</strong> secventiere. Se urmareste<br />

<strong>in</strong>itierea unui veritabil tri<strong>al</strong> pentru <strong>in</strong>vestigarea cancerului de prostata, pre<strong>cu</strong>m <strong>si</strong><br />

stabilirea cer<strong>in</strong>telor etice, leg<strong>al</strong>e <strong>si</strong> a reglementarilor <strong>in</strong>dispensabile cercetarii<br />

medic<strong>al</strong>e care implica subiecti umani, respectand cer<strong>in</strong>tele <strong>in</strong>ternation<strong>al</strong>e. Subl<strong>in</strong>iem<br />

faptul ca <strong>in</strong>vestigatiile pre<strong>cu</strong>m <strong>si</strong> modelele matematice ce se vor stabili vizeaza<br />

tumora m<strong>al</strong>igna a prostatei depistata <strong>si</strong> netratata.<br />

Identificarea markerilor mole<strong>cu</strong>lari proteici utili diagnosti<strong>cu</strong>lui <strong>si</strong> prognosti<strong>cu</strong>lui <strong>si</strong><br />

an<strong>al</strong>izati pr<strong>in</strong> metoda imunohistochimica (TGF, p53, EGFR, VEGF, p63, AMACR) ar<br />

putea <strong>in</strong> viitor sa <strong>in</strong>lo<strong>cu</strong>iasca testul cla<strong>si</strong>c PSA. Investigarea markerilor genetici<br />

<strong>in</strong>formativi (BRCA1, BRCA2, P53, EGFR) <strong>al</strong>aturi de anom<strong>al</strong>iile cromozomi<strong>al</strong>e (Ch 8p)<br />

pr<strong>in</strong> utilizarea LOH, MSI, hibridizare <strong>in</strong> <strong>si</strong>tu, vor putea <strong>in</strong>tra <strong>in</strong> <strong>al</strong>catuirea unui panel de<br />

<strong>in</strong>vestigatie folo<strong>si</strong>tor nu numai prognozarii progre<strong>si</strong>ei <strong>si</strong> recidivelor, cat <strong>si</strong> pentru<br />

evidentierea susceptibilitatii famili<strong>al</strong>e.<br />

Proiectul nostru i<strong>si</strong> propune de asemeni <strong>in</strong>terpretarea biostatistica a rezultatelor<br />

obt<strong>in</strong>ute pr<strong>in</strong> aplicarea de nomograme <strong>si</strong> modele matematice specifice care sa<br />

permita prognozare la nivel <strong>in</strong>dividu<strong>al</strong> pre<strong>cu</strong>m <strong>si</strong> stabilirea unui <strong>al</strong>goritm de ev<strong>al</strong>uare<br />

a stadiului <strong>si</strong> prognosti<strong>cu</strong>lui.<br />

Bazandu-ne pe experienta celor mai importante cl<strong>in</strong>ici de urologie d<strong>in</strong> Bu<strong>cu</strong>resti (P1,<br />

P2 <strong>si</strong> P3), pe expertiza <strong>in</strong> diagnostic mole<strong>cu</strong>lar a partenerilor d<strong>in</strong> sfera cercetarii<br />

biologice (CO) <strong>si</strong> pe aplicabilitatea modelelor statistice <strong>si</strong> <strong>si</strong>mularilor (P4 <strong>si</strong> P5),<br />

proiectul propune, nu <strong>in</strong> ultimul rand, stabilirea unei baze de date <strong>si</strong> un pachet de<br />

lucru util medicilor practicieni <strong>in</strong> abordarea cancerului de prostata.<br />

Propunerea noastra de proiect se <strong>in</strong>cadreaza <strong>in</strong> obiectivele programului 4, directia<br />

de cercetare sanatate (4.1). Mi<strong>si</strong>unea <strong>proiectului</strong> este de <strong>in</strong>tegrare a metodelor de<br />

<strong>in</strong>vestigatie <strong>si</strong> <strong>in</strong>tervention<strong>al</strong>e bazate pe medic<strong>in</strong>a mole<strong>cu</strong>lara <strong>si</strong> celulara, asociate<br />

cancerului de prostata, <strong>in</strong> scopul de a facilita aplicarea <strong>in</strong> Romania de strategii<br />

profilactice <strong>si</strong> terapeutice moderne pentru aceasta afectiune a carei <strong>in</strong>cidenta <strong>si</strong><br />

prev<strong>al</strong>enta este <strong>in</strong> crestere.<br />

Ne propunem formularea <strong>si</strong> verificarea de ipoteze pre<strong>cu</strong>m <strong>si</strong> elaborarea unor teorii<br />

priv<strong>in</strong>d carc<strong>in</strong>ogeneza <strong>in</strong> cancerul de prostata. Urmarim aplicarea tehnologiilor de<br />

laborator moderne pe materi<strong>al</strong>ele furnizate de cele mai importante cl<strong>in</strong>ici de<br />

urologie d<strong>in</strong> Bu<strong>cu</strong>resti. Con<strong>si</strong>deram ca fi<strong>in</strong>d un obiectiv important an<strong>al</strong>iza statistica <strong>si</strong>
