Procesul verbal (fig.2) conţine date despre utilizatorul de la care se prelevează proba de apă uzată inclusiv despre branşmentul de apă şi racordul de canalizare. Raportul de prelevare (fig.3) conţine date despre proba de apă uzată prelevată şi consemnarea opţiunii utilizatorului pentru prelevarea sau nu a probei martor.
