UTA Management Drug & Alcohol Policy<br />
To report a medication, <strong>the</strong> employee’s physician (a licensed doctor familiar<br />
with <strong>the</strong> employee’s medical history <strong>and</strong> job duties) must read <strong>and</strong> sign <strong>the</strong><br />
Medication Approval Form. Employees should bring a Medication Approval<br />
Form with <strong>the</strong>m when <strong>the</strong>y visit <strong>the</strong>ir doctor. This form must be returned to<br />
Human Resources within 7 days from <strong>the</strong> date of return to work, or from <strong>the</strong><br />
start of taking <strong>the</strong> medication(s), whichever is earlier. Forms may be h<strong>and</strong> –<br />
delivered, mailed or faxed (287-4555) to HR.<br />
Employees have <strong>the</strong> responsibility to explain <strong>the</strong>ir job duties to <strong>the</strong>ir doctor <strong>and</strong><br />
ensure that use of prescribed medications will not pose a safety risk to<br />
<strong>the</strong>mselves, o<strong>the</strong>r employees, or <strong>the</strong> general public.<br />
If <strong>the</strong> employee’s use of a prescription or over-<strong>the</strong>-counter drug has endangered<br />
<strong>the</strong> employee, o<strong>the</strong>r employees or <strong>the</strong> public, or has contributed to an accident,<br />
<strong>the</strong> employee will be subject to discipline, including termination, under UTA<br />
policy.<br />
2) Confidentiality of Records <strong>and</strong> Verification<br />
Medication Approval Forms will be kept in a confidential medical file in Human<br />
Resources <strong>and</strong> may be verified by UTA’s medical advisors. If an employee’s<br />
personal physician <strong>and</strong> UTA’s medical advisor differ regarding use of a<br />
medication, <strong>the</strong> Human Resources Department will work with <strong>the</strong> employee’s<br />
personal physician <strong>and</strong> <strong>the</strong> Authority’s medical advisor to resolve <strong>the</strong><br />
disagreement.<br />
3) Side Effects<br />
Employees who experience side effects or do not feel fit for duty, regardless of<br />
medications or previous approvals, must consult <strong>the</strong>ir personal physician <strong>and</strong><br />
immediately refrain from performing hazardous activities, including driving <strong>and</strong><br />
working with machinery.<br />
Section 3.02 Collective Bargaining Agreement<br />
To <strong>the</strong> extent permitted by law, drug testing of bargaining unit employees is also governed by <strong>the</strong><br />
Letters of Agreement dated April 22, 1985 <strong>and</strong> December 19, 1994.<br />
