Summary: A seven-page guide which urges patients to get involved in <strong>the</strong>ir medical<br />

treatment by asking questions of <strong>the</strong> health care provider. The guide lists<br />

<strong>the</strong> questions that patients should ask <strong>and</strong> includes various tips for patients<br />
