Theme Two. Inter-relationship between symptoms<br />

When describ<strong>in</strong>g how their sleep quality affected them, all participants described the<br />

effects of poor sleep quality on their other FMS symptoms, such as a pa<strong>in</strong>, fatigue <strong>and</strong><br />

their ability to manage to those symptoms. Complex <strong>in</strong>teractions between sleep <strong>and</strong><br />

symptoms became evident, with participants report<strong>in</strong>g both direct <strong>and</strong> <strong>in</strong>direct effects of<br />

sleep quality on their other symptoms, <strong>and</strong> a bi-directional effect between sleep <strong>and</strong><br />

pa<strong>in</strong>.<br />

Effect of sleep on pa<strong>in</strong><br />

Eleven participants felt that a poor nights sleep was directly associated with reduced<br />

overall function<strong>in</strong>g the follow<strong>in</strong>g day. When asked to describe a typical day liv<strong>in</strong>g with<br />

FMS one participant described;;<br />

“Today I‟m just exhausted, but I didn‟t sleep, I hardly slept last night so<br />

erm yeah, they (symptoms) completely vary, erm the pa<strong>in</strong>‟s worse today, but<br />

that‟s because I‟m tired, it‟s always worse when I‟m tired.” [011 F59]<br />

Wak<strong>in</strong>g up due to the pa<strong>in</strong><br />

Participants also described that pa<strong>in</strong> affected their sleep quality. Sitt<strong>in</strong>g or ly<strong>in</strong>g for long<br />

periods of time was perceived to lead to <strong>in</strong>creased pa<strong>in</strong> dur<strong>in</strong>g the night <strong>and</strong> participants<br />

often highlighted that they needed to keep chang<strong>in</strong>g positions <strong>in</strong> order to rema<strong>in</strong><br />

comfortable <strong>in</strong> bed, or they were restless at night as they were unable to feel<br />

comfortable <strong>in</strong> one position. Many participants stated that be<strong>in</strong>g <strong>in</strong> pa<strong>in</strong> woke them up<br />

dur<strong>in</strong>g the night.<br />

“Lack of sleep affects your pa<strong>in</strong> <strong>and</strong> er, <strong>and</strong> if you‟re <strong>in</strong> a lot of pa<strong>in</strong> <strong>and</strong><br />

you roll over, the pa<strong>in</strong> will wake you up.” [014 F53]<br />

Reduced ability to manage symptoms<br />

The majority of participants felt that the quality of their sleep was related to feel<strong>in</strong>gs of<br />

fatigue <strong>and</strong> pa<strong>in</strong>. Three participants elaborated that they believed that feel<strong>in</strong>g tired after<br />

a poor nights sleep made it harder to cope the follow<strong>in</strong>g day, affect<strong>in</strong>g their perceptions<br />

of pa<strong>in</strong>.<br />
