<strong>Sleep</strong> <strong>and</strong> Fibromyalgia Syndrome<br />

Information Sheet<br />

Thank you for your <strong>in</strong>terest <strong>in</strong> this study on sleep <strong>and</strong> Fibromyalgia Syndrome.<br />

Please read the follow<strong>in</strong>g <strong>in</strong>formation carefully <strong>and</strong> feel free to ask me if anyth<strong>in</strong>g is<br />

unclear or if there is anyth<strong>in</strong>g else you would like to know about the study. Please take<br />

time to decide if you would like to participate, although you may withdraw from this<br />

study at any time.<br />

The study<br />

This study is <strong>in</strong>vestigat<strong>in</strong>g the beliefs people with Fibromyalgia have about their<br />

sleep, their sleep patterns, <strong>and</strong> their experience of Fibromyalgia Symptoms.<br />

The study will be conducted by myself (Alice Theadom), a Health Psychologist<br />

work<strong>in</strong>g, under the supervision of Mark Cropley at the University of Surrey <strong>and</strong> Kirsty-<br />

Louise Humphrey, a postgraduate student at the University of Surrey.<br />

Instructions<br />

If you wish to participate please complete both copies of the consent form attached,<br />

return<strong>in</strong>g one copy to us with your questionnaire <strong>and</strong> reta<strong>in</strong><strong>in</strong>g one copy for your<br />

yourself. You will be given a questionnaire to complete at your own pace. It is 10 pages<br />

long <strong>and</strong> takes approximately 15-20 m<strong>in</strong>utes to complete. There will be detailed<br />

<strong>in</strong>structions on how to complete each section of the questionnaire. There are no right or<br />

wrong answers to these questions <strong>and</strong> you may choose not to respond to any<br />

questions you do not wish to answer.<br />

Benefits <strong>and</strong> Risks<br />

Participat<strong>in</strong>g <strong>in</strong> this study will contribute to the underst<strong>and</strong><strong>in</strong>g of Fibromyalgia. It is<br />

hoped that the f<strong>in</strong>d<strong>in</strong>gs of the study will help to provide <strong>in</strong>formation for practitioners <strong>in</strong><br />

the field, with the aim to improve the quality of life for <strong>in</strong>dividuals with this health<br />

condition.<br />

You may f<strong>in</strong>d answer<strong>in</strong>g questions about your illness or some specific questions<br />

are upsett<strong>in</strong>g or distress<strong>in</strong>g for you. You are under no pressure to participate <strong>in</strong> the<br />

study <strong>and</strong> you may choose not to answer specific questions or to withdraw from the<br />

study at any time.<br />

This study has received ethical approval from by the departmental ethics<br />

committee. All <strong>in</strong>formation will be completely anonymous <strong>and</strong> confidential. Please<br />

return all questionnaires by April 25 th 2005.<br />
