<strong>Sleep</strong>, Cop<strong>in</strong>g <strong>and</strong> Quality of Life<br />

Information Sheet<br />

Thank you for your <strong>in</strong>terest <strong>in</strong> this study on sleep, cop<strong>in</strong>g <strong>and</strong> quality of life.<br />

Please read the follow<strong>in</strong>g <strong>in</strong>formation carefully <strong>and</strong> feel free to ask me if anyth<strong>in</strong>g is<br />

unclear or if there is anyth<strong>in</strong>g else you would like to know about the study. Please take<br />

time to decide if you would like to participate, although you may withdraw from this<br />

study at any time.<br />

The study<br />

This study is <strong>in</strong>vestigat<strong>in</strong>g the beliefs people with have about their sleep, their sleep<br />

patterns <strong>and</strong> quality of life. The study will be conducted by myself (Alice Theadom), a<br />

Chartered Health Psychologist <strong>and</strong> Dr Mark Cropley at the University of Surrey.<br />

Instructions<br />

If you wish to participate please complete the attached questionnaire , return<strong>in</strong>g this to<br />

us <strong>in</strong> the envelope provided. Please complete the questionnaire <strong>in</strong> your own time <strong>and</strong><br />

at your own pace. It is 9 pages long <strong>and</strong> takes approximately 15-20 m<strong>in</strong>utes to<br />

complete. There will be detailed <strong>in</strong>structions on how to complete each section of the<br />

questionnaire. There are no right or wrong answers to these questions <strong>and</strong> you may<br />

choose not to respond to any questions you do not wish to answer.<br />

Benefits <strong>and</strong> Risks<br />

Participat<strong>in</strong>g <strong>in</strong> this study will contribute to the underst<strong>and</strong><strong>in</strong>g of sleep, cop<strong>in</strong>g <strong>and</strong><br />

quality of life <strong>in</strong> Fibromyalgia syndrome by compar<strong>in</strong>g responses between patients with<br />

<strong>and</strong> without this condition. It is hoped that the f<strong>in</strong>d<strong>in</strong>gs of the study will help to provide<br />

<strong>in</strong>formation for practitioners <strong>in</strong> the field, with the aim to improve the quality of life for<br />

<strong>in</strong>dividuals with this health condition.<br />

You may f<strong>in</strong>d answer<strong>in</strong>g some of the questions upsett<strong>in</strong>g or distress<strong>in</strong>g for you. You<br />

are under no pressure to participate <strong>in</strong> the study <strong>and</strong> you may choose not to answer<br />

specific questions or to withdraw from the study at any time.<br />

All <strong>in</strong>formation will be completely anonymous <strong>and</strong> confidential. Please return all<br />

questionnaires <strong>in</strong> the stamped addressed envelope provided. This study has received<br />

ethical approval from the University of Surrey ethics committee.<br />
