5.11.1 From time to time, BellSouth may require access to OTI’s Collocation Space.<br />

BellSouth retains the right to access OTI’s Collocation Space for the purpose of<br />

making BellSouth equipment and building modifications (e.g., installing, altering or<br />

removing racking, ducts, electrical wiring, HVAC, and cabling). In such cases,<br />

BellSouth will give notice to OTI at least forty-eight (48) hours before access to<br />

OTI’s Collocation Space is required. OTI may elect to be present whenever<br />

BellSouth performs work in the OTI’s Collocation Space. The Parties agree that<br />

OTI will not bear any of the expense associated with this type of work.<br />

5.11.2 In the case of an emergency, BellSouth will provide oral notice of entry as soon as<br />

reasonably practicable after such entry.<br />

5.11.3 OTI must provide the local BellSouth Central Office Building Contact with two<br />

(2) Access Devices that will allow BellSouth entry into any enclosed and locked<br />

Collocation Space including, but not limited to, an Adjacent Arrangement,<br />

pursuant to the requirements contained in this Section.<br />

5.12 OTI’s Access<br />

5.12.1 Pursuant to Section 12 below, OTI shall have access to its Collocation Space<br />

twenty-four (24) hours a day, seven (7) days a week. OTI agrees to provide the<br />

name and social security number, date of birth, or driver’s license number of each<br />

employee, supplier, or agent of OTI or OTI’s Guest(s) with OTI’s written request<br />

for access keys or cards (Access Devices) for specific BellSouth Premises, prior to<br />

the issuance of said Access Devices, using Form RF-2906-C, the “CLEC and<br />

CLEC Certified Supplier Access Request and Acknowledgement” form. The<br />

appropriate key acknowledgement forms (the “Collocation Acknowledgement<br />

Sheet” for access cards and the “Key Acknowledgement Form” for keys) must be<br />

signed by OTI and returned to BellSouth Access Management within fifteen (15)<br />

days of OTI’s receipt of these forms. Failure to return these properly<br />

acknowledged forms will result in the subsequent access key or card requests<br />

being held by BellSouth until the proper acknowledgement documents have been<br />

received by BellSouth and reflect current information. Charges for the Security<br />

Access System and for Security Access Devices will be billed at the rates set forth<br />

in Exhibit B. Access Devices may not be duplicated under any circumstances.<br />

OTI agrees to be responsible for all Access Devices and for the return of all<br />

Access Devices in the possession of OTI’s employees, suppliers, agents, or Guests<br />

after termination of the employment relationship, the contractual obligation with<br />

OTI ends, upon the termination of this Agreement, or upon the termination of<br />

occupancy of Collocation Space in a specific BellSouth Premises. OTI shall pay<br />

all applicable charges associated with lost or stolen Access Devices.<br />

5.12.2 BellSouth will permit one (1) accompanied site visit, which will be limited to no<br />

more than one (1) hour, to OTI’s designated Collocation Space, after receipt of<br />

the BFFO, without charge to OTI. OTI must submit to BellSouth the completed<br />

Access Control Request Form for all employees, suppliers, agents or Guests<br />
