Grant: Unraveling and treating impaired pain inhibition during exercise in patients with Myalgic<br />
Encephalomyelitis/Chronic Fatigue Syndrome: a clinical experiment targeting brain neurotransmission.<br />

Abstract: In a previous MERUK-funded study, we showed that pain inhibition during exercise is impaired in patients with myalgic encephalomyelitis /<br />
chronic fatigue syndrome. This finding is in line with our previous findings <strong>of</strong> delayed pain inhibition at rest in response to heat pain. It becomes<br />
more and more clear that the lack <strong>of</strong> pain inhibition accounts in part for various symptoms at rest and following exercise in particular ('postexertional<br />
malaise'). However, the mechanisms behind the lack <strong>of</strong> pain inhibition during exercise remain to be revealed.<br />
The mechanisms responsible for impaired pain inhibition in response to exercise in people with MF/CFS might be crucial in developing appropriate<br />
drug treatments to prevent post-exertional malaise. The present study aims at examining whether a drug targeting brain neurotransmission is able<br />
to decrease post-exertional malaise in patients with inhibitory pathways.<br />
In addition, the recognition <strong>of</strong> ME/CFS as a severe illness remains an important issue. Therefore, the present study proposal aims at comparing the<br />
results obtained in ME/CFS patients with those obtained in a patient group with an established medical diagnosis characterized by chronic pain <strong>of</strong><br />
peripheral rather than central origin. To compare the findings in ME/CFS with those obtained from RA patients, the project includes 2 triple blind,<br />
randomized controlled, cross-over studies.<br />
