About the Organization: Central New Hampshire VNA<br />
& Hospice (CNHVNAH) is a non-pr<strong>of</strong>it home health,<br />
hospice and community service agency serving<br />
Belknap County, Southern Carroll County and several<br />
surrounding communities. We provide home based<br />
pr<strong>of</strong>essional healthcare and supportive care services<br />
using skilled, licensed and certified clinicians through<br />
our four programs: Homecare, Hospice & Palliative<br />
Care, Young Family Pediatric Care, and Senior Support Services. CNHVNAH is the regional provider <strong>of</strong><br />
Medicare-certified hospice services, infusion therapy and pediatric homecare service.<br />
The organization is licensed by the state <strong>of</strong> New Hampshire and certified by Medicare and Medicaid.<br />
Although some services receive reimbursement from insurance companies, other badly needed services are<br />
dependent on community and grant support. In particular, our Pediatric, Senior Support Services and<br />
Hospice & Palliative Care programs rely on charitable contributions in order to be sustained. It is for this<br />
reason that we request funding from the communities we serve including <strong>Meredith</strong>. We leverage town<br />
contributions with donations from individuals, community service organizations and agency fund raising<br />
events. Funds provided by the town <strong>of</strong> <strong>Meredith</strong> are used to serve <strong>Meredith</strong> residents.<br />
Below you will find a summary <strong>of</strong> the services provided in <strong>Meredith</strong> during the past year.<br />
Visit Data: During the past year, CNHVNAH provided 168 <strong>Meredith</strong> residents with 3071 in-home visits<br />
by pr<strong>of</strong>essionals or support service workers and 75 vaccine services to children and adults.<br />
Hospice & Palliative Care: Last year saw a particularly significant increase in palliative care/hospice<br />
services - with over 1,800 visits given to 35 <strong>Meredith</strong> residents at the end <strong>of</strong> life, representing a 31%<br />
increase over the previous year and a 200% increase from 2009.<br />
Pediatric Services: 125 visits were made to 35 <strong>Meredith</strong> families with young children to provide guidance<br />
and support to new parents, connect children with a medical home, provide direct healthcare services or<br />
<strong>of</strong>fer parental guidance & education.<br />
Senior Support Services: 366 visits were made to 16 frail elderly in <strong>Meredith</strong> to help maintain them in a<br />
home setting rather than transfer to a nursing home.<br />
Homecare Services: 752 visits were made to 41<strong>Meredith</strong> residents recovering from illness or managing a<br />
chronic illness at home.<br />
It is a privilege to provide services to <strong>Meredith</strong> residents, and we are grateful for the town support that<br />
enables us to carry on these health-related community functions.<br />
