AN ORDINANCE to enter into an agreement with the District Advisory
Council of Cuyahoga County Ohio General Health District for the provision of
public health services within the City of Lakewood in an amount not to exceed

one year period commencing January 1 2010

WHEREAS the Cuyahoga County Board of Health has proposed providing
public heath services for residents of the City of Lakewood at a per capita rate of
370 and
WHEREAS it is necessary to continue public health services to residents of the
City of Lakewood through the District Advisory Council of Cuyahoga County Ohio
General District Now Therefore
BE IT ORDAINED BY THE CITY OF LAKEWOOD STATE OF OHIO
Sectiori L That the Mayor on behalf of the City is hereby authorized
to enter into an
agreement with the District Advisory Council of Cuyahoga
County Ohio General Health District for the provision ofi public health services
within the City of Lakewood in an amount not to exceed Two Hundred Nine
Thousand Five Hundred Ninety Dollars209
590 00 for a one year period
commencing Januaryl ZO10 pursuant to the authority granted in Section
3709 07 of the Ohio Revised Code
Section 2 It is found and determined that all formal actions of this Council
concerning and relating to the passage of this ordinance were adopted in an open
meeting of this Council and that alsuch deliberations of this Council and of any of its
committeesthat resulted in such formal action were in meetings open to the public in
compliance with all legal requirements
