AN ORDINANCE to enter into an agreement with the District Advisory<br />
Council of Cuyahoga County Ohio General Health District for the provision of<br />
public health services within the City of Lakewood in an amount not to exceed<br />

one year period commencing January 1 2010<br />

WHEREAS the Cuyahoga County Board of Health has proposed providing<br />
public heath services for residents of the City of Lakewood at a per capita rate of<br />
370 and<br />
WHEREAS it is necessary to continue public health services to residents of the<br />
City of Lakewood through the District Advisory Council of Cuyahoga County Ohio<br />
General District Now Therefore<br />
BE IT ORDAINED BY <strong>THE</strong> <strong>CITY</strong> <strong>OF</strong> <strong>LAKEWOOD</strong> STATE <strong>OF</strong> OHIO<br />
Sectiori L That the Mayor on behalf of the City is hereby authorized<br />
to enter into an<br />
agreement with the District Advisory Council of Cuyahoga<br />
County Ohio General Health District for the provision ofi public health services<br />
within the City of Lakewood in an amount not to exceed Two Hundred Nine<br />
Thousand Five Hundred Ninety Dollars209<br />
590 00 for a one year period<br />
commencing Januaryl ZO10 pursuant to the authority granted in Section<br />
3709 07 of the Ohio Revised Code<br />
Section 2 It is found and determined that all formal actions of this Council<br />
concerning and relating to the passage of this ordinance were adopted in an open<br />
meeting of this Council and that alsuch deliberations of this Council and of any of its<br />
committeesthat resulted in such formal action were in meetings open to the public in<br />
compliance with all legal requirements<br />
