What is LTF?<br />
Long term follow-up is the continuation of your child’s health care beyond the<br />
active treatment for childhood cancer. <strong>The</strong> aim of this care is:<br />
• to assess the impact of cancer and treatment on your child’s physical, social<br />
and psychological well-being<br />
• to enhance your child’s quality of life by managing current and ongoing<br />
health issues, and to minimize the risk of future health complications<br />
• to support your child, and your family, in their return to school, community<br />
and lifestyle<br />
• to provide <strong>information</strong> to you about the potential delayed effects of the<br />
disease and treatment<br />
• to gather <strong>information</strong> about your child’s health and well-being after treatment,<br />
which may influence cancer care for other children and young people in the<br />
future.<br />
Who is eligible for LTF?<br />
As the treatments for childhood cancer become more effective and the number of<br />
survivors increases, we are learning more about the after-effects of these therapies.<br />
Every child or young person who has lived through cancer needs to know about<br />
the effects of their experience so that they can make informed health care choices.<br />
Do I have to come to hospital?<br />
This care can be provided in a variety of locations, depending on your child’s<br />
diagnosis and the intensity of the treatment received. Some children or young<br />
people will need to attend specialized clinics in the Children’s <strong>Cancer</strong> Centres,<br />
at either <strong>The</strong> Royal Children’s Hospital or Monash Medical Centre, whilst others<br />
will be able to have investigations and follow-up care provided by a GP or<br />
<strong>Paediatric</strong>ian in their own community.<br />
What are the benefits of LTF?<br />
<strong>The</strong> LTF program focuses on:<br />
• health promotion and awareness<br />
• early detection and intervention of cancer-related or other health and wellbeing<br />
issues<br />
• the provision of accurate and relevant education and <strong>information</strong><br />
• empowering young people and families to take charge of their health care<br />
in a supportive environment.<br />
