...and going home: what to take<br />
<strong>The</strong>re’s a lot to remember before your child is discharged from hospital.<br />
<strong>The</strong> nurses on the ward, your child’s Nurse Coordinator and doctors will<br />
organise most things for you. Here’s a checklist. Feel free to copy it and<br />
use it to make sure you have everything done when it is time to go home.<br />
A Appointments<br />
Do you have a follow-up appointment?<br />
Do you know the arrangements for your child’s next admission?<br />
B Blood Card<br />
Do you have your blood card?<br />
Where will you have your next blood test taken?<br />
Do you know what day and time to have them taken?<br />
Do you have a request slip for your next test?<br />
C Chemotherapy Plan<br />
Do you know where and when your next treatment will be?<br />
Will it be give in the Day Oncology Unit or will you be staying overnight?<br />
How many days?<br />
D ‘Drugs’ or medicines<br />
Do you need to collect medications from pharmacy before you go home?<br />
Are you running low on any of your medications?<br />
Do you have any questions about any of your medications?<br />
E Emergency letter & everything else<br />
Have you got a CCC emergency letter?<br />
When and where will your child next have their Hickman line/port flushed<br />
and what arrangements have been made?<br />
Is your child having medications via an insulfon and what arrangements<br />
have been made?<br />
Do you need any equipment at home ie nasogastric supplies, tapes,<br />
insulfons?<br />
If you are not sure about your ABCDEs please speak to your CCC<br />
