6Physiology and Pathophysiology of Blood Cellsendothelium of the body cavities; epithelioid cells; foreign body macrophages(including Langhans’ giant cells); and many other cells.Lymphocytes are divided into two major basic groups according tofunction.Thymus-dependent T-lymphocytes, which make up about 70% oflymphocytes, provide local defense against antigens from organic and inorganicforeign bodies in the form of delayed-type hypersensitivity, as classicallyexemplified by the tuberculin reaction. T-lymphocytes are dividedinto helper cells and suppressor cells. The small group of NK (naturalkiller) cells, which have a direct cytotoxic function, is closely related to theT-cell group. The other group is the bone-marrow-dependent B-lymphocytes or B-cells, which make up about 20% of lymphocytes. Through their developmentinto immunoglobulin-secreting plasma cells, B-lymphocytes are responsiblefor the entire humoral side of defense against viruses, bacteria, and allergens.Table 1Cells in a normal peripheral blood smear and their physiological rolesCell type Function Count(% of leukocytes)Neutrophilic bandgranulocytes (bandneutrophil)Neutrophilic segmentedgranulocyte (segmentedneutrophil)Lymphocytes(B- and T-lymphocytes,morphologically indistinguishable)MonocytesEosinophilic granulocytesBasophilic granulocytesPrecursors of segmented cellsthat provide antibacterialimmune responsePhagocytosis of bacteria;migrate into tissue for this purposeB-lymphocytes (20% of ⎫lymphocytes) mature and ⎪form plasma cells antibody⎪⎪production.⎬T-lymphocytes (70%): cyto-⎪toxic defense against viruses, ⎪foreign antigens, and tumors. ⎭Phagocytosis of bacteria, protozoa, fungi, foreign bodies.Transformation in target tissueImmune defense against parasites, immune regulationRegulation of the response tolocal inflammatory processes0–4%50–70%20–50%2–8%1–4%0–1%
