150Erythrocyte and Thrombocyte AbnormalitiesIn unexplained anemia, thrombocytopenia, or leukocytopenia, anypossible triggers (Table 25) must be discontinued or avoided. Panmyelophthisisor aplastic anemia is an acute disease that can only be overcomewith aggressive treatment (glucocorticoids, cyclosporin, antilymphocyteglobulin).Bone Marrow Carcinosis and Other Space-OccupyingProcessesAnemia resulting from bone marrow infiltration by growing, spaceoccupyingtumor metastases can in principle be normochromic. However,under the indirect influence of the underlying disease, it tends more oftento be hypochromic (secondary anemia).Normoblasts in the differential blood analysis (Fig. 9 a, p. 33) particularlysuggest the possibility of bone marrow carcinosis, because their presenceimplies destruction of the bone marrow–blood barrier. Usually, bone marrowcarcinosis leads eventually to lower counts in other cell series,especially thrombocytes.Bone metastases from malignant tumors rarely affect the bone marrowand hematopoiesis, and if they do, it is usually late. The most commonmetastases in bone marrow derive from small-cell bronchial carcinomaand breast cancer.In the differential diagnosis, the effects of direct bone marrow infiltrationmust be distinguished from phenomena caused by microangiopathichemolytic anemia (MHA) in the presence of tumor (p. 144). Carcinosis andMHA may of course coexist.Bone marrow cytology in these situations tends to reveal a generallydecreased density of hematopoietic cells and signs of reactive marrow asseen in secondary anemia (p. 134). Only in a few field views—often at theedge of the smear—will one occasionally encounter atypical cell elementswhich cannot be assigned with certainty to any of the hematopoietic blastfamilies. The critical feature is their close arrangement in clusters. Theseatypical cells are at least as large as myeloblasts or proerythroblasts (e.g.,in small-cell bronchial carcinoma), usually considerably larger. Tumortype cannot be diagnosed with certainty (except, e.g., melanoma). Bonemarrow histology and possibly immunohistology tests must be performedif there is any doubt, or in the case of negative cytological findingsor dry tap, since the clustered, focal character of metastases naturallymeans that they may not be obtained in every aspirate.
