Best initial test: Tap <strong>the</strong> joint (arthrocentesis). A finding of > 50,000 white<br />

cells is consistent with infection.<br />

Gram stain has a 50–60 percent sensitivity.<br />

Treatment<br />

Empiric <strong>the</strong>rapy with ceftriaxone and vancomycin intravenously is effective.<br />

This is <strong>the</strong> choice for CCS when you have to write in one answer.<br />

The following table shows o<strong>the</strong>r possible answers for multiple-choice questions.<br />

These are used in combination: one for Staphylococcus/Streptococcus and one<br />

for Gram-negative bacilli.<br />

differential diagnosis.<br />

This condition is often asymptomatic. It may lead to pain, stiffness, aching,<br />

and fractures. Soft bones lead to bowing of <strong>the</strong> tibias. Sarcoma arises in 1<br />

percent of patients.<br />
