up. You do not have to do anything for <strong>the</strong>m to come. Test results on CCS are like<br />

your phone bill: you do not have to do anything for your bills to arrive; <strong>the</strong>y automatically<br />

show up as time passes.<br />

When do I transfuse packed red blood cells?<br />

Answer:<br />

• Hematocrit < 30 in an older person<br />

• Hematocrit < 20–25 in younger patients with no heart disease<br />

When do I transfuse fresh frozen plasma?<br />

Answer: When <strong>the</strong>re is elevated prothrombin time/INR and vitamin K is too slow.<br />

When do I transfuse platelets?<br />

Answer: If <strong>the</strong> patient is bleeding or to undergo surgery, transfuse platelets when <strong>the</strong>y<br />

are < 50,000.<br />

What is <strong>the</strong> most common cause of death in GI bleeding?<br />

Answer: Myocardial ischemia. That is why you should get an EKG in older patients with<br />

severe GI bleeding. The myocytes of <strong>the</strong> left ventricle cannot distinguish between ischemia,<br />

anemia, carbon monoxide poisoning, and coronary artery stenosis. All of <strong>the</strong>se<br />

lead to myocardial infarction.<br />

When is “nasogastric tube” <strong>the</strong> answer?<br />

Answer: Place a nasogastric (NG) tube in <strong>the</strong> occasional patient when you are unsure<br />

whe<strong>the</strong>r bleeding is from an upper or lower gastrointestinal source. The nasogastric<br />

tube has no <strong>the</strong>rapeutic benefit; it will not stop bleeding. Iced saline lavage is worthless<br />

and is always wrong.<br />
