Necrotic Pancreatitis<br />

Diagnostic Testing<br />

In <strong>the</strong> past, Ranson’s criteria were <strong>the</strong> major method of determining <strong>the</strong> severity<br />

of pancreatitis. Ranson’s criteria are operative criteria to see who needs<br />

pancreatic debridement. The CT scan effectively replaces Ranson’s criteria as<br />

<strong>the</strong> most precise method of determining severity.<br />

Treatment<br />

When <strong>the</strong> CT shows > 30 percent necrosis of <strong>the</strong> pancreas, <strong>the</strong> patient should<br />

receive antibiotics such as imipenem; and<br />

··<br />

undergo CT-guided biopsy.<br />

If <strong>the</strong> biopsy shows infected, necrotic pancreatitis, <strong>the</strong> patient should have<br />

surgical debridement of <strong>the</strong> pancreas.<br />

Hepatitis<br />

Patients with acute hepatitis will all present is a very similar way. You cannot<br />

accurately determine <strong>the</strong> etiology of acute hepatitis from history and presentation<br />

alone. All patients present with:<br />

• Hepatitis B is associated<br />

with polyarteritis nodosa<br />

(PAN) in 30 percent of<br />

cases.<br />

• Hepatitis C is associated<br />

with cryoglobulinemia.<br />

Jaundice<br />

Fatigue<br />

Weight loss<br />

Dark urine caused by bilirubin in <strong>the</strong> urine<br />

Hepatitis B and C are more likely to present with serum sickness-phenomena<br />

like joint pain, urticaria, and fever.<br />

Hepatitis E is most severe in pregnant women. It can be fatal.<br />

Diagnostic Testing<br />

All patients with acute hepatitis will give an elevated conjugated (direct) bilirubin.<br />

This will lead to bilirubin in <strong>the</strong> urine or urobilinogen. Uncon-jugated<br />

bilirubin, such as that associated with hemolysis, is not water soluble and will<br />

not pass into <strong>the</strong> urine. Unconjugated bilirubin is attached to albumin.<br />

Viral hepatitis gives an elevated ALT level.<br />
