MPH Student Checklist<br />

Congratulations on joining our program at the Colorado School of Public Health! Now that you have decided to<br />

pursue your public health education with us, we’d like to share with you the next steps in the process. Official<br />

acceptance into ColoradoSPH requires the completion and return of all items found in the New Student Checklist<br />

below prior to matriculation. If you have any questions please feel free to contact us at any time. We are thrilled<br />

to have you here and are happy to help along the way.<br />

REQUIRED FOR ALL CSU MPH STUDENTS<br />

Pay the tuition deposit<br />

1) $200 non-refundable tuition deposit, applied to your first semester’s tuition<br />

2) Choose the School of Public Health from the dropdown menu and submit your tuition deposit online<br />

here: https://www.cusys.edu/admissions/nelnet/AMC.html<br />

Background Check and Immunization Records through CastleBranch<br />

1) Please read and sign the Disclosure and Authorization <strong>for</strong>m, which is available here.<br />

2) Email the signed Disclosure <strong>for</strong>m to Coloradosph.admissions@ucdenver.edu.<br />

•All attachments should be PDFs, and named .<br />

3) After we receive your Disclosure and Authorization <strong>for</strong>m, we will send you an email with instructions<br />

<strong>for</strong> you to order your background check. Please note you are responsible <strong>for</strong> the cost of the background<br />

check. The cost of the background check varies. If you have completed a background check within<br />

CastleBranch in the last 3 years, you do not need to complete a new background check.<br />

4) Immunization Records, as well as a signed copy of the ColoradoSPH Student Code of Conduct, must be<br />

submitted through the Compliance Tracker within your CastleBranch portal. Immunization requirements<br />

and the Student Code of Conduct can be found in your CastleBranch portal. Immunization records and the<br />

signed Student Code of Conduct should be submitted prior to beginning coursework at our school.<br />

Complete the Tuition Classification Form<br />

1) ALL students must complete and submit the electronic tuition classification <strong>for</strong>m. Instructions can be<br />

found on page 18.<br />

2) If you are applying <strong>for</strong> WRGP (see below), you need to complete both the tuition classification <strong>for</strong>m and<br />

the WRGP <strong>for</strong>m.<br />

In addition to the items above, please complete the following if they apply to your situation:<br />

• If you are a current resident in one of the following states: Alaska, Arizona, Cali<strong>for</strong>nia, Hawaii, Idaho,<br />

Montana, Nevada, New Mexico, North Dakota, Oregon, South Dakota, Utah, Washington, Wyoming, and<br />

Commonwealth of the Northern Mariana Islands (CNMI), you are eligible to receive in-state tuition to<br />

attend ColoradoSPH. Please submit the WRGP Verification Form. Please note that students cannot claim<br />

the WRGP benefit and establish Colorado domicile at the same time.<br />

• If applicable, submit FAFSA <strong>for</strong> financial aid. All financial aid requests, regardless of your home campus,<br />

should indicate the University of Colorado (Anschutz Medical Campus). In<strong>for</strong>mation regarding financial aid<br />

can be found on our <strong>website</strong>. (Please select the Anschutz Medical Campus).<br />

Once these items are processed*, you will receive an email containing in<strong>for</strong>mation about registering <strong>for</strong> classes. CSU and<br />

UNC students are required to meet with an advisor prior to registering <strong>for</strong> courses.<br />

*Processing can take some time, so please submit all documents as soon as possible to avoid delays in course<br />

registration.<br />

***International Students*** – The matriculation process outlined above must be completed be<strong>for</strong>e we can begin the I-<br />

20 process. Please reach out to us at ColoradoSPH.admissions@ucdenver.edu, to let us know thatyou will be applying<br />

<strong>for</strong> F1 Visa status. More in<strong>for</strong>mation about the cost of attendance can be found here.<br />
