el origen de la proteinuria y de la enfermedad subyacente,
incluyendo los posibles agentes infecciosos. Es
necesario realizar una anamnesis completa, una exploración
física exhaustiva y las análisis laboratoriales y de
PCR para detectar enfermedades infecciosas. Es importante
descartar la presencia de cualquier agente infeccioso,
ya que el tratamiento de algunas enfermedades
glomerulares puede ser inmunosupresor, lo que empeoraría
cualquier infección no diagnosticada.
La nefritis asociada a la enfermedad de Lyme es un
ejemplo de una enfermedad presumiblemente de etiología
infecciosa (Borrelia burgdorferi) que se ha asociado con
glomerulonefritis membranoproliferativa, rápidamente
progresiva y concurrente con necrosis tubular severa,
inflamación corticointersticial y uremia aguda. Posiblemente
la patogénesis principal de la enfermedad sea la
formación de inmunocomplejos. El pronóstico en estos
casos se considera reservado, debido a la naturaleza
rápida y progresiva de la enfermedad, aunque el soporte
con una terapia inmunosupresora podría ser útil (24).
■ Conclusión
En resumen, el riñón puede verse afectado , mediante de
una variedad de mecanismos, por las enfermedades
infecciosas. En algunos casos, la infección tiene su origen
en el riñón, pero en otros, el riñón se ve afectado por
las repercusiones sistémicas de la enfermedad infecciosa.
En muchos casos de infección asociada al fallo
renal agudo, una antibioterapia adecuada puede eliminar
la causa, ofreciendo por tanto un pronóstico más favorable
en comparación con otros casos de fallo renal agudo o
de la enfermedad renal crónica, que suelen ser progresivos
y con frecuencia de etiología desconocida.
