VanKerrebroekAccepted for: Cardiorhythm 2007, 2-4 February2007, Hong KongBackground: Little has been published on Far-Field R signals in ICD!sMethods: We screened device-saved IECG!s tolook for the presence of Far-Fieled R signals. Ina total of 20 patients we retrieved 72 IECG!s.Only 5 IECG!s (6,4%) documented ventricularpaced events.We defined Far-Field R any signal on the atrialchannel with peak to peak amplitude of at least0,2mV with onset falling within –50ms and +100ms from the Ventricular Event Marker.Results: In 75% of the IECG!S Far-Field R was documented,all after ventricular sense. The mean Far-Field R amplitude was 0,61 mV with a STDDEV of0,39mV. Ventric. event to Far-Field R onset variedfrom –50 to + 90 ms. Only 6 IECG!s showed a positiveFar-Field R onset. Far-Field R signal durationvaried from 40 to 140 ms. 26% of the Far-FieldR signals have been seen at least intermittentlyby the device. In 9 (4 patients) of these 14 situationsthis led to inappropriate mode switches.Programming the Far-R suppression algorithm!§ON!.. with appropriate duration resolved theinappropriate mode switches.Conclusions: 1. In 75% of the IECG!s Far-FieldR was documented. Only 26% of these were atleast intermittently seen by the device. In 17%of the situations inappropriate mode switcheswere documented.2.
