Intel® Health Guide and Chronic Diseases<br />

Of Interest To<br />

Healthcare organizations providing ongoing care for patients with one or more chronic or longterm<br />

conditions, particularly patients who have multiple conditions and are frequently<br />

hospitalized. This may include organizations such as health payors, hospitals, home health<br />

organizations, community services and physician groups. In the UK, it includes Primary Care<br />

Trusts and Health Authorities.<br />

Challenge<br />

Chronic or long-term conditions (LTCs) have surpassed accidents and communicable diseases<br />

to become the leading cause of death worldwide. According to the World Health Organization,<br />

chronic disease accounts for 60 percent of all deaths, and half of the 35 million people who died<br />

from chronic disease in 2005 were younger than 70. 1 Chronic conditions include coronary heart<br />

disease (CHD), congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD),<br />

diabetes, stroke, hypertension, asthma, cancer and others.<br />

Chronic disease accounts for 70 percent of all deaths in the U.S., or 1.7 million annually. 2<br />

Almost half of all Americans – 133 million people in 2005 – have at least one chronic condition,<br />

and the medical care costs of people with chronic diseases consumes more than 75 percent of<br />

US medical care costs. 3<br />

In England 15.4 million people, including 60 percent of those over age 60, suffer from a longterm<br />

condition. People with LTCs account for 72 percent of all inpatient bed days, 65 percent of<br />

all outpatient appointments and 52 percent of all GP appointments. The Department of Health<br />

estimates that the treatment and care of patients with LTCs consume 69 percent of England’s<br />

primary and acute care budget. 4<br />

Opportunity<br />

Many people with chronic diseases want to do more to improve their health. When England’s<br />

Department of Health commissioned a survey of 1,000 participants at a National Citizens’<br />

Summit 5, 86 percent expressed a desire for local physician practices to provide more support to<br />

help them take care of their own health and well-being. Half of those with LTCs were not aware<br />
