own approach or passing vessel and<br />

comes off vessel. Inexperienced<br />

L/Master. Best lee not made or speed<br />

inappropriate for conditions and ship does<br />

not achieve the requested heading<br />

(through getting into irons or<br />

misjudgement of helm and engine speed<br />

required to affect turn) or alters<br />

speed/course substantially during the<br />

launches approach. Steering or<br />

propulsive failure on launch. Seasickness<br />

or fatigue impairs judgment of<br />

launchmaster. L/Master misjudges effects<br />

of interaction between vessels causing<br />

heavy landing which knocks pilot off his<br />

feet. Pilot misjudges timing of transfer<br />

to/from launch in adverse sea conditions.<br />

Pilot not secured during transfer to<br />

foredeck or the tether parts. Pilot<br />

disconnects too early and does not<br />

maintain a handgrip when on foredeck.<br />

Pilot launch comes away from ships side<br />

through adverse sea conditions or launch<br />

master misjudgement. Pilot ladder parts<br />

through becoming caught under launch<br />

belting during rise and fall of launch<br />

(ladder may be in poor condition).<br />

Pilots are required to conform to STCW-<br />

95 medical requirements for seafarers.<br />

Historically Wellington pilots safety<br />

record has been good. Lighter<br />

displacement pilot launches are in use<br />

which provide a less stable platform<br />

than those previously in service but<br />

techniques are used to pin launch<br />

alongside during transfers. Pilots are<br />

trained to adopt best lee during transfers<br />

and personally arrange this rather than<br />

leave it to other party. Pilots wear LSA<br />

at all times in transfer. Hazard may also<br />

apply to official passengers such as<br />

MAF personnel boarding special ships -<br />

transfer is always on basis of prior<br />

safety briefing and utmost safety but risk<br />

still applies.
