Social Worker: Member Association of Certified Social Workers (ACSW).<br />

Application Procedure<br />

The Counselor or therapist, at the request of the qualifying client who is receiving the<br />

counseling, is to make application for a grant to the Chairperson of the Committee on Clergy<br />

Care in the following manner:<br />

1. The application and brochure are available on the conference website: www.nccumc.org,<br />

Ministries tab, Ministerial Relations tab and Downloads, then Clergy Care tab, or the<br />

counselor or therapist may request an application form and instructions from the Office of<br />

Ministerial Relations. This request should be addressed in care of the Office of Ministerial<br />

Relations, 700 Waterfield Ridge Place, Garner, <strong>NC</strong> 27529, or by calling either 919-779-<br />

6115 or 800-849-4433, ext 244.<br />

2. Once the application and instructions for a Clergy Care Grant are received by the<br />

counselor/therapist, and a determination is made that the client qualifies, the application<br />

should be completed and returned to the Office of Ministerial Relations for processing. To<br />

protect the confidentiality of the client, the client’s name and social security number shall<br />

never appear on the application. When more than one member of the client’s family is<br />

involved in counseling, all family members shall be assigned the same case number.<br />

3. The counselor/therapist will receive the financial aid grant on behalf of the client to apply<br />

to the client’s account or to refund to the client if the account has already been paid.<br />

Clients should request from the therapist a statement showing the amount of financial<br />

assistance received from the Clergy Care Committee and should request the counselor/<br />

therapist to contact the Chairperson of Clergy Care to negotiate an acceptable procedure<br />

if any problems arise concerning the process. The client, not the Clergy Care Committee,<br />

is responsible for all contracts, negotiated fees and payment of services that are provided.<br />

4. The deadline for submitting grant applications is in late December of the calendar year in<br />

which the services were provided.<br />

In conference year 2011-12, the Committee on Pastoral Care granted $10,665.38 for<br />

counseling sessions. This represents 398 sessions with counselors and therapists during the<br />

calendar year. Grants processed in 2011-12 totaled 69, representing 259 sessions for clergy, 26<br />

for cleric and spouse, 92 for spouse, 20 for children and 1 for clergy family together. Of these<br />

60 grants, 11 received the maximum of $400.00 in assistance and 14 received at least half to<br />

two-thirds of the maximum grant.<br />
