Dx: Acute Lymphocytic Leukemia.<br />

Comment:<br />

The WBC <strong>for</strong> this patient is within the normal range,<br />

however, the presence of blast (immature) cells eaUed<br />

iymphoblasts is diagnostic of leukemia.<br />

Example El8: 3/25/91. Hema. Rpt.: Hgb.-20.3 gms%. WBC-9650. Plat.-938,000.<br />

Dx: Polycythemia vera.<br />

Example El9: 8/1/91. Hema. Rpt.: Hgb. and Hct. values low; WBC and retic, normal.<br />

Comment: This report is not diagnostic of leukemia and probably would not be a<br />

reportable case. Hematologic findings generally are not recorded <strong>for</strong><br />

other than leukemia and lymphoma cases.<br />

Patients with the diagnosis of leukemia often have a hematologic examination per<strong>for</strong>med daily.<br />

To report the findings of each examination would be excessive. The best procedure is to extract<br />

pertinent findings from the first report upon admission and the most definitive report prior to the<br />

beginning of treatment. In addition, 'reports indicating remission or relapse may be recorded as part<br />

of the follow-up in<strong>for</strong>mation.<br />
