Example F10: 5/5/91. Bronchoscopy: Pharynx, V/C, trachea, carina and left<br />

bronchus--normal. RUL bronchus and (R)<br />

bronchus intermedius occluded. Bx's taken.<br />

Imp: Carcinoma involving RUL and (R) bronchus<br />

intermedius.<br />

Comments:<br />

a. From the in<strong>for</strong>mation on this report, you know that a pathology report should exist<br />

describing the three biopsies.<br />

b. Friable means easily pulverized or crumbled.<br />

c. RUL means right upper lobe; WNL means within normal limits.<br />

Example Flh 6/10/91. Esophagoscopy: Larynx and pharynx normal. Carcinoma of<br />

esophagus at 18 cm. Bx taken.<br />

Comments:<br />

a. Not much that you can abstract from this report, but it does confirm the diagnosis and<br />

establish the location of the lesion at 18 cm which is in the upper esophagus. You should<br />

check <strong>for</strong> the presence of a pathology report, since a biopsy was taken.<br />

b. You will note that the esophagoscopy report appears on a bronchoscopy report <strong>for</strong>m. If<br />

both procedures are required, they may be done at the same time, as in Example F12.<br />

Example F12: 11/18/91. Esophagoscopy and Bronchoscopy:<br />

Constricting, friable area of esophagus at 27 cm. Bx taken. Larynx,<br />

pharynx, trachea, carina and (R) bronchus--normal. Dx: Carcinoma of<br />

esophagus.<br />

Comment:.. In this and previous reports, the diagnostic statement is based on a clinical<br />

impression. As part of most endoscopic examinations, one or more biopsies will be<br />

taken to confirm or negate this impression.<br />
