Example El2:<br />

12/2/91. Brain Scan: Imp: Abnormal brain scan compatible with subdural disease<br />

on right.<br />

Comments<br />

a. Record date.<br />

b. Record procedure per<strong>for</strong>med--Brain scan.<br />

c. Record impression along with any in<strong>for</strong>mation which seems to clarify or add to the report.<br />

d. A scan report will seldom contain a positive statement regarding the presence of cancer.<br />

Scan findings should be supported by other more definitive procedures. Thus, when you<br />

encounter an abnormal scan or isotope tracer report, look <strong>for</strong> other reports which support<br />

or negate the findings.<br />

Example El3: 3/6/91. Liver Scan: Abnormal scan consistent with a central lesion as well as<br />

irregularity along (R) lower margin. Imp: High possibility of<br />

met. disease.<br />

Comment: The various entries in the top portion of the report describe features of the<br />

examination procedure. They should not be reported.<br />

Example El4:<br />

10/28/91. Bone scan: Areas of abnormality in skull, neck, sternum, shoulders, ribs,<br />

spine, pelvis, and left femur.<br />
