are entitled to Early and Periodic Screen<strong>in</strong>g, Diagnosis, and Treatment<br />

(EPSDT), which is a formal vision, hear<strong>in</strong>g, and developmental<br />

screen<strong>in</strong>g. All treatment for medically necessary services identified dur<strong>in</strong>g<br />

the course <strong>of</strong> an EPSDT screen<strong>in</strong>g are covered.<br />

HUSKY healthcare is either free or low-cost, depend<strong>in</strong>g on family size<br />

and <strong>in</strong>come. The program has two parts, HUSKY A and HUSKY B.<br />

HUSKY A is free and covers low-<strong>in</strong>come children and their parents or a<br />

caretaker relative. HUSKY B is for higher <strong>in</strong>come families but only<br />

covers un<strong>in</strong>sured children up to age 19. HUSKY B requires small copayments<br />

and may also require a monthly premium depend<strong>in</strong>g on <strong>in</strong>come<br />

and family size. Once enrolled <strong>in</strong> HUSKY, families choose a<br />

Managed Care Organization (health plan) to coord<strong>in</strong>ate the benefits package.<br />

Each health plan <strong>of</strong>fers a network <strong>of</strong> participat<strong>in</strong>g doctors, dentists,<br />

hospitals, pharmacies, and other specialty providers <strong>that</strong> families must<br />

use. Mental health and substance abuse services are available to HUSKY<br />

members through the CT Behavioral Health partnership. For more <strong>in</strong>formation<br />

Medicaid (Title XIX)<br />

Medicaid is a jo<strong>in</strong>t federal/state health <strong>in</strong>surance program adm<strong>in</strong>istered<br />

by the state Department <strong>of</strong> Social Services (DSS). Medicaid provides<br />

coverage for a comprehensive array <strong>of</strong> health services and medical needs,<br />

<strong>in</strong>clud<strong>in</strong>g services not covered by Medicare. Some services require prior<br />

authorization from DSS.<br />

Medicaid encompasses both managed care for <strong>in</strong>come-eligible children<br />

and their parents under HUSKY A as well as fee-for-service coverage<br />

for adults with disabilities and persons 65 and over. People who receive<br />

State Supplement for the Aged, Bl<strong>in</strong>d or Disabled automatically qualify<br />

for Medicaid. If an applicant’s <strong>in</strong>come is not from this source, assistance<br />

under other medical coverage categories may still be possible. If<br />

an applicant’s <strong>in</strong>come is too high to qualify under normal <strong>in</strong>come eligibility<br />

guidel<strong>in</strong>es, eligibility for medical assistance may still be possible<br />

if the medical bills are very high. The asset eligibility criteria for this<br />

program vary. Applications are made through any local DSS <strong>of</strong>fice listed<br />

under Multiservice Agencies <strong>in</strong> this book.<br />

(ALMB) Programs<br />

QMB, SLMB, and ALMB, adm<strong>in</strong>istered by the Department <strong>of</strong> Social<br />

Services (DSS), are federal and state-funded <strong>programs</strong> <strong>that</strong> provide health<br />

care coverage and f<strong>in</strong>ancial assistance <strong>in</strong> pay<strong>in</strong>g Medicare costs for certa<strong>in</strong><br />

Medicare beneficiaries. QMB pays all Medicare premiums,<br />

deductibles and co-payments – up to approved rates – for eligible <strong>in</strong>dividuals.<br />

It was designed to fill the gaps <strong>in</strong> Medicare coverage by elim<strong>in</strong>at<strong>in</strong>g<br />

out-<strong>of</strong>-pocket expenses for Medicare-covered services. For those<br />

eligible for QMB there should be no cost for any Medicare-covered services<br />

from medical providers who participate <strong>in</strong> both the Medicare and<br />

Medicaid plans. The SLMB program pays the Medicare Part B premium<br />

only. The ALMB program also pays the full amount <strong>of</strong> the Medicare<br />

Part B premium per month. However, fund<strong>in</strong>g for the ALMB program<br />

is limited, so it is important to apply as soon after January 1 as<br />

possible each year. Once fund<strong>in</strong>g runs out, applications will not be accepted.<br />

These <strong>programs</strong> are available at no cost to Medicare beneficiaries<br />

(both elderly and those with disabilities). To be eligible for any <strong>of</strong><br />

the <strong>programs</strong>, you must be a citizen or legal resident <strong>of</strong> the United States<br />

and the State <strong>of</strong> Connecticut; be enrolled or eligible to enroll <strong>in</strong> Medicare<br />

Part A; have limited <strong>in</strong>come; and have limited assets. Call DSS for<br />

Medical Program (SAGA Medical)<br />

The Department <strong>of</strong> Social Services (DSS) regional <strong>of</strong>fices may provide<br />

medical assistance (SAGA Medical) to persons who are unable to pay<br />

for necessary medical expenses such as doctor visits, prescriptions, and<br />

the cost <strong>of</strong> hospitalizations. If the applicant is hospitalized and cannot<br />

afford to pay, the hospital’s bill<strong>in</strong>g department should be <strong>in</strong>formed as<br />

soon as possible and requested to file an application for SAGA Medical<br />

Children<br />

Children with Special Health Care Needs<br />

Program (CSHCN)<br />

CSHCN is a statewide program for children with chronic illness or disability.<br />

There are five Regional Medical Home Support Centers<br />

(RMHSC) provid<strong>in</strong>g <strong>in</strong>formation and support to families <strong>in</strong> their own<br />

community. RMHSCs <strong>of</strong>fer care coord<strong>in</strong>ation; resource l<strong>in</strong>kage to support<br />

groups, agencies and other <strong>help</strong>ful services; educational supports;<br />

and respite fund<strong>in</strong>g when available. The centers work with Primary Care<br />

Providers (Medical Homes) <strong>in</strong> identify<strong>in</strong>g children with special health<br />

care needs and partner<strong>in</strong>g with them to <strong>help</strong> families receive comprehensive<br />

coord<strong>in</strong>ated care. CSHCN can pay for certa<strong>in</strong> medical goods<br />

and services <strong>that</strong> <strong>in</strong>clude but are not limited to: hear<strong>in</strong>g aids, wheelchair,<br />

and special nutritional formulas for families who meet the family <strong>in</strong>come<br />

Prevention and Control Program (CLPPCP)<br />

CLPPCP conducts statewide surveillance <strong>of</strong> children tested for lead poison<strong>in</strong>g.<br />

This <strong>in</strong>cludes the collection <strong>of</strong> demographic, medical, and environmental/dwell<strong>in</strong>g<br />

data on children affected by lead poison<strong>in</strong>g. Staff<br />

evaluates the quality and appropriateness <strong>of</strong> exist<strong>in</strong>g risk reduction edu-<br />
