Eligibility Screen<strong>in</strong>g Program (CHOICES)<br />

CHOICES provides Eldercare Locator Services, which gives Connecticut<br />

residents access to an extensive network <strong>of</strong> organizations serv<strong>in</strong>g<br />

older <strong>people</strong> throughout the United States. Callers should have the name,<br />

address and zip code <strong>of</strong> the older person seek<strong>in</strong>g assistance and a brief<br />

Municipal Agent<br />

Each town and city <strong>in</strong> Connecticut has a municipal agent who will assist<br />

<strong>in</strong> locat<strong>in</strong>g services and provide <strong>help</strong> <strong>in</strong> complet<strong>in</strong>g program applications.<br />

Senior Centers<br />

Senior Centers <strong>in</strong> Connecticut vary <strong>in</strong> scope and number <strong>of</strong> <strong>programs</strong><br />

and services <strong>of</strong>fered. Most municipalities have multi-purpose Senior<br />

Centers which provide counsel<strong>in</strong>g; health services; nutrition; education;<br />

recreational activities; transportation; wellness <strong>programs</strong>; volunteer opportunities;<br />

and other supportive services. Specific program <strong>in</strong>formation<br />

may be obta<strong>in</strong>ed by contact<strong>in</strong>g the town’s Senior Center or through<br />

Senior EyeCare Program (SEP)<br />

Formerly known as the National Eye Care Project, SEP ensures <strong>that</strong> every<br />

senior has access to medical eye care. The program is designed for<br />

<strong>in</strong>dividuals age 65 and older who have not seen an ophthalmologist <strong>in</strong> 3<br />

years or more; are not <strong>in</strong> a doctor-restricted health plan; or do not have<br />

VA benefits. The program is free for the un<strong>in</strong>sured. For the <strong>in</strong>sured,<br />

Medicare or private <strong>in</strong>surance is billed and the balance is waived. Eligible<br />

seniors receive a comprehensive medical eye exam and treatment<br />

for any disease diagnosed at the time <strong>of</strong> the <strong>in</strong>itial visit for up to one<br />

year. Eyeglasses, prescriptions, hospital services and fees <strong>of</strong> other medical<br />

Veterans<br />

Connecticut Department <strong>of</strong> Veterans’ Affairs<br />

The Department <strong>of</strong> Veterans’ Affairs was established to provide expanded,<br />

<strong>in</strong>tegrated, and coord<strong>in</strong>ated services to Connecticut veterans and their<br />

families. The agency provides comprehensive assistance to veterans, their<br />

spouses, or eligible dependents <strong>in</strong> protect<strong>in</strong>g their rights and privileges<br />

and obta<strong>in</strong><strong>in</strong>g the benefits to which they are entitled under federal, state<br />

and local laws. Counsel is provided to veterans concern<strong>in</strong>g educational<br />

tra<strong>in</strong><strong>in</strong>g and retra<strong>in</strong><strong>in</strong>g; health, medical and rehabilitation services; hous<strong>in</strong>g<br />

assistance; and benefits for veterans <strong>in</strong> nurs<strong>in</strong>g homes. The agency<br />

Soldiers’, Sailors’, and Mar<strong>in</strong>es’ Fund<br />

The Soldiers’, Sailors’, and Mar<strong>in</strong>es’ Fund provides temporary assistance<br />

to veterans or their dependents who demonstrate need for food,<br />

cloth<strong>in</strong>g, medical aid, shelter, assistance <strong>in</strong> pay<strong>in</strong>g utility bills, or other<br />

temporary f<strong>in</strong>ancial service. The veteran must have served <strong>in</strong> the Armed<br />

Forces <strong>of</strong> the United States dur<strong>in</strong>g a wartime period, have an honorable<br />

discharge, and be a legal resident <strong>of</strong> the State <strong>of</strong> Connecticut. Wartime<br />

Benefits available to veterans <strong>in</strong>clude educational assistance, home loan<br />

guaranties, disability compensation, pension, medical care, life <strong>in</strong>surance<br />

Dependency and Indemnity Compensation: available to spouses, parents,<br />

and children <strong>of</strong> service personnel or veterans who die from a disease<br />

or <strong>in</strong>jury <strong>in</strong>curred on duty or, under certa<strong>in</strong> circumstances, who<br />

suffered total service-connected disability before death. VA’s death pension<br />

may also be paid to surviv<strong>in</strong>g spouse and children under 18 (23, if<br />

attend<strong>in</strong>g school) if family <strong>in</strong>come does not exceed certa<strong>in</strong> limits. Children<br />

who become permanently <strong>in</strong>capable <strong>of</strong> self- support after age 18<br />

may receive the benefit, as long as the disability rema<strong>in</strong>s and they do not<br />

marry. An additional allowance is payable to a surviv<strong>in</strong>g spouse who is a<br />

patient <strong>in</strong> a nurs<strong>in</strong>g home or otherwise determ<strong>in</strong>ed to be <strong>in</strong> need <strong>of</strong> regular<br />

aid and attendance, or who is permanently housebound because <strong>of</strong> a<br />
