CHAPTER 7: IMPLEMENTATION<br />
YEAR 3<br />
A. Adjust payment rate structure (and risk arrangements) as necessary.<br />
B. Continue emphasis on DCYF workforce development, DCYF/Medicaid<br />
program development and pre-service and in-service training with a focus on<br />
family centered practice, cultural competency and gender specific<br />
programming.<br />
C. Continue the transition <strong>of</strong> youth in traditionally contracted and POS<br />
contracted out-<strong>of</strong>-home care to comprehensive care networks.<br />
D. Continue the design and implement evaluation <strong>of</strong> Comprehensive Care<br />
Networks for out-<strong>of</strong>-home care.<br />
E. Design and implement evaluation <strong>of</strong> Comprehensive Care Networks for home<br />
and community-based services.<br />
F. Begin longitudinal study <strong>of</strong> children and youth involved in Comprehensive<br />
Care Networks.<br />
G. Issue annual system <strong>of</strong> care progress report, ensuring that family members<br />
and other key stakeholders have adequate input into the report compilation<br />
process.<br />
YEAR 4<br />
A. Refine fiscal and management reporting.<br />
B. Continue evaluation <strong>of</strong> Comprehensive Care Networks for out-<strong>of</strong>-home<br />
services.<br />
C. Transition children and families receiving home and community-based<br />
services at point <strong>of</strong> entry to Comprehensive Care Networks.<br />
D. Continue evaluation <strong>of</strong> Comprehensive Care Networks for home and<br />
community-based services.<br />
E. ssue annual system <strong>of</strong> care progress report, ensuring that family members<br />
and other key stakeholders have adequate input into the report compilation<br />
process.<br />
YEAR 5<br />
A. Produce comprehensive system <strong>of</strong> care evaluation report including but not<br />
limited to analyses <strong>of</strong> access, services utilization, quality and performance<br />
measures, and cost-effectiveness.<br />
B. Revise system <strong>of</strong> care design/implementation based on evaluation findings<br />
and recommendations.<br />
C. Issue annual system <strong>of</strong> care progress report, ensuring that family members<br />
and other key stakeholders have adequate input into the report compilation<br />
process.<br />
