SAMPLE IMPLEMENTATIONAppendix AA.1 INTRODUCTIONThe <strong>Afghanistan</strong> <strong>Mortality</strong> <strong>Survey</strong> <strong>2010</strong> (AMS <strong>2010</strong>) was the first nationwide survey of its kind.A nationally representative sample of 24,000 households was selected. All women 12-49 who were usualresidents of the selected households or who slept in the selected households the night before the surveywere eligible for the survey. The survey was expected to result in about 28,800 interviews of women 12-49. The main objectives of the AMS <strong>2010</strong> were to provide national estimates of (maternal) mortality,causes and determinants of mortality for adults, children, and infants by age, sex, wealth status, and otherkey socioeconomic background variables. The survey also provides insight into risk factors that may haveprecipitated death, such as low utilization and coverage of basic health services, low socio-economicstatus, and other determinants. Finally, the survey also provides other demographic indicators includinglife expectancy, crude birth and death rates, and direct estimates of under-5 mortality rates, adult mortalityrates, the maternal mortality ratio, and total fertility rates.The survey was designed to produce representative estimates for most of the indicators for thecountry as a whole, for the urban and the rural areas separately, and for each of the three survey domainswhich are regroupings of the eight geographical regions. The compositions of the domains are givenbelow:• The North, which combines the Northern region and the North Eastern region, consists of 9provinces: Badakhshan, Baghlan, Balkh, Faryab, Jawzjan, Kunduz, Samangan, Sari Pul andTakhar.• The Central, which combines the Western region, the Central Highland region and the Capitalregion, consists of 12 provinces: Badghis, Bamyan, Daykundi, Farah, Ghor, Hirat, Kabul,Kapisa, Logar, Panjsher, Parwan and Maydan Wardak.• The South, which combines the Southern region, the South Eastern region and the Easternregion, consists of 13 provinces: Ghazni, Hilmand, Kandahar, Khost, Kunar, Laghman,Nangarhar, Nimroz, Nuristan, Paktika, Paktya, Uruzgan and Zabul.The AMS <strong>2010</strong> used four questionnaires:• A Household Questionnaire that was administered in all sampled households to collectinformation on household composition, housing characteristics and household possessions;and identify all deaths that occurred in the preceding five years before the survey;• Each identified death that occurred in the selected household in the three years before thesurvey was followed up with one of three Verbal Autopsy Questionnaires, depending on theage at death: deaths to children 0-28 days; deaths to children 29 days-11 years; and deaths toadults age 12 years and above.• A Woman’s Questionnaire was administered to all women age 12-49 years, in order to obtainbasic socioeconomic characteristics as well as key demographic indicators such as the totalfertility rate, childhood mortality rates and utilization of maternity care services. TheWoman’s Questionnaire includes a sibling history module from which was derived adultmortality rates, and pregnancy-related mortality rates and ratios.Appendix A | 145
