Relevant documentation is available for review.St<strong>and</strong>ard F11: Service Items <strong>and</strong>/or Worksheets added to the <strong>HMIS</strong> database have entry <strong>and</strong> exitdates that accurately reflect the paper files or intake packets.Guideline F11: The agency has a Quality Assurance Plan in place <strong>and</strong> a process for verifying that entry<strong>and</strong> exit dates in the files match the <strong>HMIS</strong>.
