The final page of thedocument is a checklist, listing each document that the agency must execute <strong>and</strong> keep onfile.Participation AgreementBetween[<strong>HMIS</strong> Lead Agency Name]<strong>and</strong>[Agency Name]This agreement is entered into on _______________(dd/mm/yy) between the [<strong>HMIS</strong> Lead AgencyName], hereafter known as “HLA,” <strong>and</strong> ___________________________________(agency name), hereafter known as "Agency," regarding access <strong>and</strong> use of the [Homeless <strong>Management</strong>Information System <strong>Project</strong> ], hereafter known as "<strong>HMIS</strong>P."I. IntroductionThe <strong>HMIS</strong>P, a shared human services database, allows authorized personnel at homeless <strong>and</strong> humanservice provider agencies throughout [Geographic Region], to enter, track, <strong>and</strong> report on informationconcerning their own clients <strong>and</strong> to share information, subject to appropriate inter-agency agreements, oncommon clients.<strong>HMIS</strong>’s goals are to:• Improve coordinated care for <strong>and</strong> services to homeless persons in the [Geographic Region],• Provide a user-friendly <strong>and</strong> high quality automated records system that expedites client intakeprocedures, improves referral accuracy, <strong>and</strong> supports the collection of quality information thatcan be used for program improvement <strong>and</strong> service-planning, <strong>and</strong>• Meet the reporting requirements of the U.S. Department of Housing <strong>and</strong> Urban Development(HUD), Housing Development Authority (HDA) <strong>and</strong> other funders as needed.In compliance with all state <strong>and</strong> federal requirements regarding client/consumer confidentiality <strong>and</strong> datasecurity, the <strong>HMIS</strong> is designed to collect <strong>and</strong> deliver timely, credible, quality data about services <strong>and</strong>homeless persons or persons at risk for being homeless. The Michigan Coalition Against Homelessness(HLA) administers the <strong>HMIS</strong>P through a subcontract with the Michigan State Housing DevelopmentAuthority (HDA).II. HLA Responsibilities1. HLA will provide the Agency 24-hour access to the <strong>HMIS</strong>P data-gathering system, via internetconnection.2. HLA will provide model Privacy Notices, Client Release forms <strong>and</strong> other templates foragreements that may be adopted or adapted in local implementation of <strong>HMIS</strong>P functions.Participation Agreement 84
