2. Maternity/Parental LeavePlease review the complete details of the Employment <strong>Insurance</strong> Act (E.I.A.) and your provincialEmployment Standards Act (E.S.A.) for maternity/parental leave provisions.We will automatically extend coverage to all employees on maternity leave, up to the maximum periodallowable under the maternity provisions of the E.I.A. and/or E.S.A., unless the employee gives theEmployer written notice that the employee does not intend to continue to participate in the <strong>Plan</strong>. (Note:if the employee elects to discontinue coverage, ALL coverages will cease. The employee will NOT be giventhe opportunity to select only certain coverages to be continued/discontinued). It is not necessary toreport an employee on maternity leave to <strong>RBC</strong> <strong>Insurance</strong>, unless either:a) the employee has elected not to continue to participate in the <strong>Plan</strong>, orb) the employee has elected to extend the maternity leave of absence beyond the maximum periodallowable under the maternity provisions of the E.I.A. and/or E.S.A.If either of the above apply, please notify your local <strong>RBC</strong> <strong>Insurance</strong> sales office and include the followinginformation:■■Policy number(s)■■Division number(s)■■Full name of employee■■Employee certificate/identification number■■Reason for absence■■Start date of maternity/parental leave■■Expected date of returnWe will then confirm, in writing, the coverages and the period during which coverages may becontinued, including the date on which any coverages may terminate.Note:Your <strong>Plan</strong> provides that all insurance coverages may be continued in force, if premiumscontinue to be paid, for a period not to exceed 12 months. Short term disability and long termdisability coverages, however, will cease on the date immediately following the maximum periodallowable under the maternity provisions of the E.I.A. and/or E.S.A. if the employee elects toextend the leave of absence beyond that date.To re-apply for coverages which the employee has elected to discontinue during the employee’smaternity/parental leave of absence, refer to the “Re-Hired Employees” section in this <strong>Guide</strong>.<strong>Plan</strong> Administrator’s <strong>Guide</strong> — <strong>List</strong> <strong>Bill</strong>19
